Objective:To observe the therapeutic effect of Qingrexiaobi decoction combined with external application of Chinese medicine on acute gouty arthritis of dampness-heat accumulation,evaluate its efficacy and safety,and further explore an effective method for the treatment of acute gouty arthritis by combining traditional Chinese and western medicine.Methods:In this clinical observation,a total of 60 patients with acute attack of gout were selected,all of whom were admitted to the Department of Rheumatology and Immunology of the Second Affiliated Hospital of Shandong First Medical University from August 2017 to August 2019.The clinical observation cases were randomly divided into control group(n=30)and experimental group(n=30)by random number table method.The control group was treated with meloxicam,and the experimental group was additively treated with Qingrexiaobi prescription combined with Chinese medicine for external application.The changes of pain score,,quantitative scores of joint symptoms and signs,TCM symptom score,C-reactive protein,erythrocytic sedimentation rate and serum uric acid in the two groups before and after treatment were statistically analyzed,as well as the total clinical response rate and adverse reactions in the two groups after treatment.Results:Before treatment,there were no significant differences in gender,age,course of disease,pain score,quantitative score of joint symptoms and signs,TCM syndrome score,C-reactive protein,erythrocytic sedimentation rate and uric acid between the two groups(P>0.05).There were statistically significant differences in pain score,joint symptom and sign quantitative score,TCM syndrome score,C-reactive protein and erythrocytic sedimentation rate between 2 groups before and after treatment(P<0.05),and there were no statistically significant differences in blood uric acid level between 2 groups before and after treatment(P>0.05).After treatment,there were statistical differences in pain score,quantitative score of joint symptoms and signs,TCM syndrome score,C-reactive protein and erythrocytic sedimentation rate between the two groups(P<0.05),the experimental group was better than the control group,and there was no statistical difference in blood uric acid level between the two groups after treatment(P>0.05).After treatment,the total clinical effective rate of experimental group was 96.7%(29/30)higher than that of control group 80.0%(24/30),and there was statistical difference between groups(P<0.05).After treatment,the incidence of adverse reactions in experimental group 6.7%(2/30)was lower than that in control group 10.0%(3/30),and there was no statistical difference between groups(P>0.05).Conclusion:The clinical efficacy of the experimental group is better than the control group,which can relieve the swelling and pain of the patients more quickly.There were no serious adverse reactions in experimental group and control group,and the safety was good.Compared with western medicine alone,the treatment of acute gouty arthritis by internal and external application of Traditional Chinese medicine can benefit more patients,which is a beneficial exploration of the treatment of gout by integrated traditional Chinese and Western medicine and has the value of clinical application.
