Objective:To summarize Professor Meng Jie’s experience in treating CNAG by using the Ancient and Modern Medical Case Cloud platform to data mine and analyze the case data of patients with CNAG treated by Professor Meng Jie.Methods:A total of 201 cases that attended the outpatient clinic of Director Meng Jie of the Department of Spleen,Gastrointestinal,Liver and Gallbladder from April 2019 to January 2023 and met the western diagnostic criteria of CNAG,Chinese diagnostic criteria and the inclusion and exclusion criteria of this study were collected,the first diagnosis information was collated and extracted,the professional terminology was standardized,the pairs cooperated,and the cases were entered into the ancient and modern medical case cloud platform completely and accurately,and after standardization of data After data standardization,frequency statistics were performed on the patients’ basic information,clinical manifestations and prescription drug composition and sexual taste and meridian,and the common evidence types,common symptoms,and high-frequency drugs of mentor Meng Jie’s treatment of CNAG were statistically analyzed,and association rules,cluster analysis and complex network analysis were used to mine the correlation between evidence types and drugs,drugs and drugs of CNAG,as well as the common drug pairs of mentor’s treatment of CNAG and core prescriptions.The study results were also visualized by software for visualizing the network,exporting the data to Excel,and plotting the corresponding graphs based on the obtained data.Results:1.201 patients with CNAG were included in this study,including 133 males and 68 females,and the proportion of females was higher than that of males.The age of onset was predominantly 40-69 years old patients,accounting for 70.65%of the total cases.2.There were 142 symptoms in the subject,among which,the percentages>10%were:gastric distension,belching,acid reflux,gastric pain,insomnia,loose stools,thoughtless eating,emotional discomfort,heartburn,dry mouth and bitterness;19 types of tongue texture were involved,with light red,red and dark tongue being the most common;15 types of tongue coating were involved,with yellowish greasy coating being the most common;16 types of pulse signs were involved,with stringent and sunken pulse being the most common.3.The evidence types of CNAG in the subject were liver-depression and qi-stagnation evidence most common,followed by spleen-stomach damp-heat evidence,then liver-depression and spleen-deficiency evidence and liver-stomach depression-heat evidence.Spleen deficiency with blood stasis,spleen and stomach deficiency with cold,stomach yin deficiency,and cold-heat mismatch were relatively uncommon.4.The subject involved in Chinese medicine has Chinese medicine for 231 flavors,the cumulative frequency of drug use is 3430 times,and the 10 most frequently used drugs are,in order:licorice,Pinellia ternata,radix bupleuri,scutellaria,fructus aurantii,white peony root,poria,Codonopsis pilosula,ginger,and pericarpium citri reticulatae.5.In the analysis of the properties of Chinese medicine,the four qi were used with the highest frequency for warm drugs,followed by flat drugs and cold drugs.The five flavors are pungent drugs in the first place,followed by bitter and sweet drugs;the main channels are the spleen,lung,stomach and liver channels;the effects are mainly to dry dampness,dissolve phlegm,eliminate lumpiness and disperse knots,and tonify the spleen and benefit the qi.6.Association rule:select association rule analysis in the analysis method,select Chinese medicine in the front item and Chinese medicine in the back item to get 21 groups of Chinese medicine combinations with strong association;select evidence type in the front item and Chinese medicine in the back item to get 6 groups of combinations with strong association of evidence type and drugs.7.Cluster analysis:Hierarchical clustering was selected in the analysis method,and the top 20 most frequently occurring traditional Chinese medicines were analyzed to obtain 3 drug combinations:①Dang Shen,Hou Pu,Huang Lian,Fu Ling,Chen Pi,Citrus aurantium,Zhe Bei Mao,and Hai Tang Tang.② Glycyrrhiza glabra,Radix et Rhizoma gastrodiae,Scutellaria baicalensis.③Ginger,Chai Hu,Bai Shao,Dazao,Gui Zhi,Sheng Oyster,Citrus aurantium,Ziziphi stems,and Radix et Rhizoma.8.The core prescriptions were obtained by complex network analysis:Bupleurum,scutellaria,licorice,Fructus aurantii,pinellia pinellia,alba paeoniae and ginger.Conclusion:1.Clinically,the population with CNAG is mainly middle-aged and elderly people,mostly women,and the evidence type is mostly liver-depression-qi stagnation evidence and spleen-stomach damp-heat evidence.2.Professor Meng Jie’s use of pungent,bitter,sweet,warm,flat,and cold drugs,with the effects of drying dampness and resolving phlegm,eliminating lumpiness and dissipating knots,tonifying the spleen and benefiting the qi.His high-frequency drugs,pairs of drugs in association analysis,corner drug combinations and clustering combinations,and core prescriptions all correspond to this medication characteristic.3.Professor Meng Jie believes that the main pathogenesis of CNAG is ① emotionally motivated disease,mostly due to Qi dysregulation;②loss of spleen health,mostly due to Spleen Qi deficiency;③spleen deficiency,mostly due to Damp-Heat and Blood stasis.The treatment is based on ① dredging the liver and regulating qi,regulating qi flow,②tonifying the spleen and benefiting qi to protect the middle jiao,③clearing the stomach and bile,clearing heat and resolving phlegm,④ combining cold and heat to harmonize yin and yang,which can effectively treat the evidence related to CNAG and provide new ideas for clinical guidance of CNAG treatment. |
