Based On Data Mining To Analyze Syndrome Differentiation And Medication Rule Of Chronic Erosive Gastritis By Traditional Chinese Medicine

Part One:Based on Data Mining to Analyze Syndrome Differentiation and Medication Rule of CEG by Traditional Chinese Medicine of contemporary medical practitionersResearch Objective:In this study,by using the ancient and modern medical case cloud platform,the data mining analysis was conducted on the TCM syndrome and prescription medications of contemporary medical practitioners in the treatment of CEG,and the rules of identification and medication were summarized,in order to provide a reference basis for the treatment of CEG in Chinese medicine.Research Method:In this study,we searched and collected the articles about Chinese medicine for CEG which were publicly published during the ten-year period from September 2011 to September 2021,from the China National Knowledge Infrastructure（CNKI）,VIP China Science and Technology Journal Database（VIP）,Wanfang Data Knowledge Service Platform（WANFANG DATA）and Chinese biomedical literature service system（CBM）.Based on the inclusion and exclusion,we conducted frequency analysis,correlation analysis,cluster analysis and complex network analysis on the TCM syndrome,prescriptions,composition of the prescriptions and the four qi and five tastes of Chinese medicine,respectively.The core prescriptions for the treatment of CEG were explored to research the potential correlation between herb-medicines.Research Result:In this study,a total of 263 articles were collected and screened,including 304 prescriptions,among which302 TCM syndrome with clear evidence-based diagnosis were available,and all data involved a total of 37 TCM syndrome,247 herbal medicines,and 361 herbal effects.Among all the TCM syndromes,syndrome of dampness-heat of spleen and stomach appeared most frequently,followed by syndrome of incoordination between liver and stomach.There were 31 herb-medicines with a frequency ≥30 and the top ten in order of frequency are liquorice,atractylodes macrocephala,coptis chinensis,pinellia ternata,dried tangerine or orange peel,radices paeoniae alba,bletilla hyacinthina reichb,radix bupleuri,poria cocos,codonopsis pilosula.The classical prescriptions with the highest similarity to different combinations of high-frequency Chinese herbal medicines are Shengyang Yi Stomach Tang,Chai Hu Soothing the Liver San and Tai Shan Pan Shi San.Analysis of the properties of the herbal medicines revealed that the efficacy of the medicines were used mainly to clear heat and detoxify,dry dampness and phlegm,clear heat and dry dampness,remove fire and detoxify,and astringent and stop bleeding.The most medicines enter the spleen,stomach;the warm medicines is the most of four characters;the bitter medicines is the most of five tastes,followed by pungent,sweet medicines.The association analysis found that the frequency of liquorice was60.20%,19 pairs of associated medicine pairs were obtained after eliminating liquorice and the medicine combinations with higher co-occurrence frequencies were coptis chinensis-pinellia ternata（82times）,pinellia ternata-dried tangerine or orange peel（75times）,dried tangerine or orange peel-bighead atractylodes rhizome（73times）.The top six correlation pairs according to the confidence coefficient ranking were: codonopsis pilosula-bighead atractylodes rhizome（0.69）,cuttlebone-bletilla hyacinthina reichb（0.69）,poria cocos-bighead atractylodes rhizome（0.67）,poria cocos-pinellia ternata（0.62）,pinellia ternata-coptis chinensis（0.61）,bletilla hyacinthina reichb-bighead atractylodes rhizome（0.61）.Medicine-TCM syndrome correlations had found that pinellia ternata,scutellaria baicalensis,coptis chinensis are commonly used for syndrome of dampness-heat of spleen and stomach.The basic herbs for syndrome of incoordination between liver and stomach are radix bupleuri and radices paeoniae alba.The syndrome of weakness of the spleen and stomach mostly uses codonopsis pilosula,bighead atractylodes rhizome,dried tangerine or orange peel.Cluster analysis of 31 herbs with frequencies ≥ 30 got 5 groups of herbs:Group 1: liquorice,radices paeoniae alba;Group 2:dried tangerine or orange peel,bighead atractylodes rhizome,poria cocos;Group 3:pinellia ternata,coptis chinensis,scutellaria baicalensis;Group 4:corydalis tuber,bletilla hyacinthina reichb,cuttlebone,pseudo-ginseng,the rhizome of chuanxiong,radix bupleuri,fructus aurantii,rhizoma cyperi;Group 5:codonopsis pilosula,astragalus membranaceus,dandelion,the root of red-rooted salvia,fructus amomi,radices saussureae,evodia rutaecarpa,chinaberry fruit,mangnolia officinalis,semen coicis,fritillaria thunbergii miq,radix curcumae,fructus aurantii immaturus,curcuma zedoary,concha arcae.Finally,complex network analysis was performed on all 247 herbs and 16 backbone herbs were extracted according to clinical needs.The herbs are as follows:bighead atractylodes rhizome,liquorice,radices paeoniae alba,dried tangerine or orange peel,coptis chinensis,pinellia ternata,radix bupleuri,poria cocos,fructus aurantii,bletilla striata,codonopsis pilosula,cuttlebone,corydalis tuber,scutellaria baicalensis,evodia rutaecarpa,astragalus membranaceus.Analysis Conclusion:In the treatment of CEG,contemporary medical practitioners mainly use heat-clearing and detoxicating herbs,add some herbs for Regulating Qi,tonifying Qi and blood,invigorating the circulation of blood,strengthening the spleen and stomach properly,while the use of herb-medicines is not bound to the spleen and stomach.Treating qi and blood together,attacking and supplementing at the same time,using different herb-medicines in response to different pathological factors to identify and tailor reflected the treatment based on syndrome differentiation of Chinese medicine.Part Two:Study on the Syndrome Differentiation and Medication Rule of Professor Huang in the Treatment of CEG by Traditional Chinese Medicine based on Data MiningResearch Objective:By using the ancient and modern medical case cloud platform,the data mining and analysis of Prof.Huang’s cases of CEG treated with TCM.The TCM syndrome and medication patterns were conducted to summarize Prof.Huang’s characteristics of academic and treatment,in order to provide some new viewpoints for the clinical treatment of CEG with TCM.Research Method:From January 2021 to December 2021,117 cases of patients diagnosed with "CEG" or "chronic superficial（non-atrophic）gastritis with erosion" by gastroscopy were collected from the outpatient and inpatient departments（Huayuanshan and Fenghuang hospital areas）of the Department of Spleen and Stomach of Hubei Provincial Hospital of TCM treated by Prof.Huang.Finally,the data was recorded into the ancient and modern medical case cloud platform for analyzing.Research Result:The cases of patients involved no significant difference in the proportion of either sex,mostly aged 40-49 and50-59 years.Syndrome of dampness-heat of spleen and stomach is the most common followed by syndrome of incoordination between liver and stomach.The frequency statistics of herbs revealed that the prescriptions included in this study involved a total of 131 herbs.There are five herbs with a frequency>60,poria cocos,bletilla hyacinthina reichb,pinellia ternata,fructus amomi, dried tangerine or orange peel,included.Herbs efficacy to dry dampness and resolve phlegm is the most,followed by astringent and anti-emetic drugs and heat detoxifying drugs.The warm and the bitter herbs are the most of four characters and five tastes;while most herbs enter the spleen,lung and stomach.The association analysis found that 5 pairs of associated medicine pairs with higher co-occurrence frequencies were dandelion-bletilla hyacinthina reichb,fructus aurantii-fructus amomi,medicated leaven-dried tangerine or orange peel,cuttlebone-pinellia ternata,cuttlebone-bletilla hyacinthina Reichb.Medicine-TCM syndrome correlations had found that dandelion,scutellaria baicalensis,coptis chinensis,pinellia ternata are commonly used for syndrome of dampness-heat of spleen and stomach.The basic herbs for syndrome of incoordination between liver and stomach are fingered citron,radices paeoniae alba and radix curcumae.The syndrome of weakness of the spleen and stomach mostly uses bighead atractylodes rhizome,poria cocos,codonopsis pilosula and rhizoma dioscoreae.Five groups of core herbs were obtained after cluster analysis:Group 1:bletilla hyacinthina reichb,fritillaria thunbergii miq,dandelion;Group2:poria cocos,bighead atractylodes rhizome;Group 3:fructus amomi,fructus aurantii;Group 4:pinellia ternata,cuttlebone,mangnolia officinalis,dried tangerine or orange peel,medicated leaven;Group 5:angelica sinensis,radices paeoniae alba,fructus aurantii immaturus,radix curcumae,fingered citron.Finally,the core prescription of Prof.Huang for the treatment of ECG was obtained by complex network analysis including bletilla hyacinthina reichb,poria cocos,bighead atractylodes rhizome,fructus amomi,pinellia ternata,dried tangerine or orange peel, mangnolia officinalis,cuttlebone,fructus aurantii immaturus,fructus aurantii,medicated leaven,radix curcumae,dandelion,fingered citron.Analysis Conclusion:Prof.Huang believes that lesions in the gastric mucosa are most commonly located in the gastric sinus and the treatment of TCM should be considered in terms of attack and protection factors,combined with endoscopic microscopic identification into the deficiency of qi,blood and yin,the excess of dampness（phlegm）,heat,blood stasis and toxicity.In the treatment of CEG,supporting the healthy Qi and eliminating pathogenic factor is taken as the principle and the elimination of the causative factors is emphasized.Using heat-clearing,fire-drying and dampness-drying herbs,qi-regulating and qi-lowering herbs,spleen-healthy and stomach-healthy herbs,and ulcer-healing and astringent herbs and treating qi and blood together,regulating the spleen and stomach together,while dispersing is combined with collecting are the characteristics of Prof.Huang.
