In order to solve the problems of unreasonable allocation of medical and health resources at the grassroots level,imbalanced supply and demand of medical services,and uneven service quality,and to change the unreasonable pattern of medical treatment,as an important measure of China’s medical and health system reform,the construction of county-level medical communities aims to promote the construction of an orderly new order for medical treatment,such as graded diagnosis and treatment and two-way referral,through the integration,sharing,and balanced allocation of medical and health resources,Improve the utilization rate of medical and health resources and the performance of medical insurance fund utilization,so that people within the county can enjoy high-quality medical and health services without leaving the county,thereby meeting the growing health needs of the people.With the introduction of relevant policy documents on promoting the construction of county-level medical communities by the country,various counties and districts across the country have carried out different forms of county-level medical community construction based on local conditions.Some achievements have been made in institutional mechanism innovation,grassroots service capacity improvement,and medical insurance payment reform,and valuable experiences have emerged for reference.However,the construction of county-level medical communities itself is a complex systematic project that involves the adjustment of interests and restructuring of functions among various stakeholders such as the government,medical communities,and hospitals.Many regions are still in the pilot and promotion stages,and there are still many difficulties and problems in practice.Therefore,exploring the problems in the construction of county-level medical communities and proposing solutions and suggestions is of great practical significance.The total amount of medical resources in P County,Shandong Province has always been insufficient and unevenly distributed.In order to provide the people with higher quality,more convenient,and more economical medical and health services,P County began the construction of county-level medical communities at the end of2019.As of the end of 2022,the construction of medical communities in P County has achieved preliminary results,with improvements in the county’s medical treatment rate,total medical and health resources,and grassroots medical and health service capabilities.However,in the long run,the current situation of medical community construction in P County is still far from the early planned construction goals,and there are still multiple difficulties and obstacles in the specific practical process.By using various methods such as literature analysis,interviews,comparative analysis,and using incentive compatibility theory and collaborative governance theory as analytical tools,this paper analyzes the problems existing in the construction of P County’s medical community,and proposes solutions based on local reality.This has strong pertinence and reference value for further improving the problems faced by P County’s medical service reform.Firstly,through on-site investigation and policy documents,analyze and sort out the construction goals,measures,and overall progress of the medical community construction in P County.Secondly,based on the research and interview results,it is proposed that the current construction of the medical community in P County mainly involves weak allocation of grassroots medical resources;Deviation of service functions of medical institutions;The efficiency of bidirectional referral operation is low;The overall efficiency of medical services is not high,and the reasons for the prominent problems in the construction of P County’s medical community are analyzed.Finally,by analyzing the construction of characteristic medical communities in individual regions of China,drawing on their advanced experience and mature practices,we propose collaborative strategies to promote the construction of medical communities from multiple aspects,such as strengthening top-level design,improving policy and institutional guarantees,strengthening interest integration,promoting internal collaboration within medical communities,enhancing grassroots strength,and promoting resource flow and sharing.
