| The current "difficult and expensive medical treatment" in the medical and health field has become a common social phenomenon.The quality of medical services is uneven,the supply and demand are unbalanced,the contradiction between doctors and patients has deepened,and the premium of medicines has become more and more serious.With the launch of the medical reform in 2009,the society and the academic circles slowly turned their attention to the integration of medical resources and the reform of the three-level medical system,and the concept of medical union was put forward.At present,the compact county medical community is recognized as the most promising form of integrated medical organization.In April 2017,the General Office of the State Council issued the "Guiding Opinions on Promoting the Construction and Development of Medical Consortiums",which clearly stated that medical communities should be established in counties and gradually Realize the sharing of medical resources in the region,further improve the grass-roots service capabilities,and promote the formation of a hierarchical diagnosis and treatment model of primary diagnosis,two-way referral,emergency and slow treatment,and upper and lower linkages.This article takes Kunshan City as the research object,through literature analysis and comparative analysis of its current medical resource distribution and the current status of the medical community’s implementation process,and analyzes the existing dilemmas of promoting the county medical community,and combines the domestic advanced The medical community reform experience and combined with the interview records of people from all walks of life put forward policy recommendations for the construction of Kunshan Medical Community to optimize its reform path.There are six chapters in the article.The first two parts mainly introduce the research background and research significance,sort out the relevant literature research on the medical community at home and abroad,and explain the concept of medical community and integrated medical care,so as to study the current situation and the medical model of Kunshan City in the following article.The development trend of future medical community construction lays a theoretical foundation.The third part is mainly to conduct a qualitative analysis of the existing medical resources in Kunshan City,introduce the medical level of public hospitals and primary hospitals in Kunshan City,and promote the exploration of the medical community,and analyze the economic and locational advantages of the medical community in Kunshan City.The dual advantages of this article provide evidence for the exploration of the medical community model in the second half of the article.The fourth part mainly combines interviews and surveys to analyze the existing difficulties and difficulties of establishing a medical community in Kunshan from different angles.There are mainly four levels of difficulties: lack of top-level design,lack of efficient hospital operating mechanism,lack of effective salary mechanism,and the public The lack of a positive concept of seeking medical treatment has caused resistance to the advancement of the Kunshan Medical Community.The fifth part is mainly to select Luohu,Shaxian and Tianchang three medical communities as the current advanced experience of successful pilot establishment of medical communities in our country to analyze and summarize,and absorb the elements of establishing medical communities from them.In the last part,in view of the difficulties and difficulties mentioned above and combined with the advanced national pilot experience,it provides suggestions on the localization of Kunshan from the three aspects of government,hospitals,and personnel to build a county-level medical community,so as to create a medical community suitable for Kunshan and unique Kunshan characteristics.To strengthen the medical level of primary hospitals and improve the hierarchical diagnosis and treatment system to provide reference for follow-up research. |
