Study On The Cooperative Mechanis Of County Medical Community

The grassroots medical service network is the cornerstone of safeguarding the people’s medical rights and interests.Actively promoting the reform of medical service integration at the county level is the key measure to solve the unreasonable allocation,low quality and fragmentation of the service system of primary medical and health resources.As a new practice of grassroots healthcare reform and innovation,the county medical community reconstructs the county medical service system,integrates the resources of the county,township,and village medical service network,and achieves regional health performance.At present,some counties in China have carried out different forms of county-level medical community construction practices based on different policy environments,resource endowments,and population characteristics,and have achieved great results.However,many areas are still in the pilot and promotion stages,and there are still many obstacles in practice.Among them,the construction of county-level medical communities involves multiple entities such as the government,hospitals,and patients.Due to significant interest adjustments and functional restructuring,there are numerous difficulties in the collaborative process,which restricts the improvement of grassroots medical service levels.Therefore,exploring the collaborative governance mechanism in the construction of county-level medical communities has important practical significance.Starting from the perspective of collaborative governance theory,this article takes the W County Medical Community as the research object,analyzes the current situation of the county medical community,conducts in-depth research on its collaborative difficulties,and analyzes the deep reasons for the collaborative difficulties from four aspects: governance thinking,top-level design,interest demands,and governance resources.Finally,combined with the collaborative governance theory,a solution is proposed.At the conceptual level,enhance awareness of collaborative concepts,establish departmental collaborative relationships,and correct community values;At the institutional level,reform the payment method of medical insurance,establish a supervision and feedback system,and rectify the community order;At the level of responsibility,optimize the mechanism of power operation,establish an assessment and evaluation system,and clarify the rights and responsibilities of the community;At the structural level,the establishment and revitalization of talent mobility,the introduction of social medical institutions,and the consolidation of community structure;At the technical level,promote the sinking of digital resources,promote information integration and collaboration,facilitate community communication,and explore a feasible path for collaborative governance of medical communities,in order to effectively solve the collaborative dilemma of county-level medical communities and improve the level of grassroots medical and health services.
