Purpose: Through a retrospective analysis of 218 inpatients with liver meridian stagnation-heat type serpentine sores in the Dermatology Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,the incidence rule and clinical treatment of this disease were summarized and sorted out,in order to further improve the treatment of liver meridian stagnation-heat type serpentine by integrated traditional Chinese and Western medicine.sores for reference.Material and method: By using the electronic medical record system of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,the medical records of inpatients in the Department of Dermatology from March 2021 to February 2022 were analyzed,sorted and entered into the database.The contents include: general information(gender,age,onset season,causes of onset,past disease history,etc.),clinical manifestations(onset site,skin lesion morphology,symptoms and signs,tongue and pulse,etc.),laboratory examinations and treatment.Use SPSS26.0 statistical software to carry out statistics on the data.Results:1.General data: Among the 218 hospitalized patients with liver meridian stagnation-heat type serpentine sores,there were 91 males(41.74%)and 127 females(58.26%),with a male-tofemale ratio of 1:1.4;the youngest patient was 24 years old,and the oldest was 94 years old.The age was 64.40±11.51 years old;61 patients(27.98%)in spring,23(10.55%)in summer,81(37.16%)in autumn,and 53(24.31%)in winter,with most cases in autumn;125 patients There were 66 cases of hypertension(30.28%),35 cases of diabetes(16.06%),and 23 cases of coronary heart disease(10.55%);there were 32 cases(14.68%)of patients with clear onset factors,and most of the predisposing factors were 23 cases of fatigue(10.55%),6 cases of emotional factors(2.75%),3 cases of upper respiratory tract infection(1.38%).2.Clinical manifestations: The incidence sites of 218 patients in this study were 103 times(47.25%)in the chest,back,flank,and ribs,63 times(28.90%)on the head and face,55 times(25.23%)in the waist and abdomen,30 times(13.76%)in the upper limbs,25 times(11.47%)in the lower limbs.,22 times(10.09%)in the perineum and buttocks,21 times(9.63%)in the shoulders and neck,and the most frequently occurred in the chest,back,flanks and flanks;131 cases(60.09%)had rash and pain as the first symptoms at the same time.,70patients(32.11%)had pain as the first symptom,17 patients(7.80%)had rash as the first symptom;38 patients(17.43%)had complications,including 21 patients with herpes zoster conjunctivitis(9.63%),17 cases of herpetic keratitis(7.80%),6 cases of urinary tract infection(2.75%),5 cases of skin infection(2.29%),3 cases of constipation(1.38%),3 cases of anemia(1.38%),3 cases(1.38%)of herpes zoster iritis,and herpes zoster conjunctivitis the most;In hospitalized patients,red tongue was the most common,208 cases(95.41%),yellow tongue was the most common,88 cases(40.37%),pulse was most common,69 cases(31.65%).3.Laboratory examination: White blood cell count was normal in 108 cases(49.54%),high in32 cases(14.68%),and low in 78 cases(35.78%);lymphocyte count was normal in 113 cases(51.83%),high in 81 cases(37.16%),and low in 24 cases(11.01%);31 cases(14.22%)with normal immune function and 187 cases(85.78%)with abnormality.4.Treatment situation: In this study,198 cases of oral traditional Chinese medicine decoction were treated by traditional Chinese medicine.The top 5 efficacy distributions were heat-clearing(47.16%),blood-activating and stasis-removing(18.20%),tonifying(10.93%),and Qi-regulating(6.97%).,Tablets(5.66%);the top 5 meridians of drugs are liver meridian(21.31%),heart meridian(17.40%),lung meridian(11.64%),spleen meridian(9.83%),stomach meridian(9.19%);The medicinal tastes are bitter(42.85%),pungent(23.51%),sweet(22.13%),light(3.67%),sour(2.13%),salty(1.02%),astringent(0.18%);medicinal properties are cold(58.95%),warm(22.73%),flat(12.33%),cool(5.93%),hot(0.06%),combined with ordinary acupuncture,fire acupuncture,bloodletting at the tip of the ear,auricular acupuncture,and local treatment of TCM external treatment Method;Western medicine treats with anti-virus,pain relief,nerve nutrition,immune regulation,etc.,combined with red light and TDP magic lamp.5.Treatment results: The average skin lesion score before treatment was 6.98±1.05 points,and the average skin lesion score after treatment was 1.05±0.48 points.After treatment,the skin lesion score was significantly lower than that before treatment(P<0.05).The average pain score before treatment The pain score after treatment was 6.90±0.96,the average pain score after treatment was 1.06±0.46,and the pain score after treatment was significantly lower than that before treatment(P<0.05).In this study,27 cases were cured,185 were effective,and 6 were ineffective.For example,the total effective rate is 97.24%.Conclusion:1.In this study,the disease was most common in women,and the age of onset is 60~74 years old.It occurs frequently in autumn.It is often accompanied by one or more diseases in the past,and fatigue is the primary predisposing factor.2.In this study,the diseased site was more common in the chest,back,flanks and ribs.The first symptoms were rash and pain at the same time.The complications were mostly herpetic conjunctivitis.The tongue and pulse were mainly red tongue,yellow coating and slippery pulse.Shingles can recur.3.In this study,the oral administration of traditional Chinese medicine decoction is mainly based on heat-clearing,blood-activating and stasis-removing drugs,which are cold in nature,bitter in taste,and return to the liver and heart meridians.4.The curative effect of integrated traditional Chinese and western medicine is definite,and the prognosis of this disease is good. |
