| Objective: To observe the expression level of serum fatty acid binding protein 4(FABP4)in type 2 diabetes mellitus(T2DM)complicated with non-alcoholic fatty liver.To explore the role and significance of serum adipocyte fatty acid binding protein expression level in the clinical treatment of T2 DM complicated with NAFLD,so as to provide new ideas and research directions for the prevention,diagnosis and treatment of T2 DM complicated with NAFLD.Methods : A total of 108 patients with type 2 diabetes who were hospitalized in the Affiliated Hospital of Guilin Medical College from April2020 to January 2021 were selected.According to the clinical data and imaging results,the subjects were divided into the T2 DM group and the T2 DM combined with NAFLD group.116 healthy adults who underwent routine physical examination in the physical examination center of our hospital during the same period were divided into NAFLD group and healthy control group according to clinical data and imaging results.Record all the subjects of alanine aminotransferase(ALT),aspartate aminotransferase(AST),urea nitrogen,creatinine,uric acid,fasting plasma glucose(FPG),fasting insulin(FINS),glycosylated hemoglobin(Hb A1c),triglycerides(TG),serum total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),insulin resistance index(HOMA-IR),and other indicators,using ELISA method to detect the serum level of FABP4,The difference of FABP4 level in each group was analyzed,and SPSS software was used for data processing and statistical analysis.Results: 1.Compared with the control group,the body weight,abdominal circumference,BMI,fasting blood glucose,insulin resistance index and hemoglobin A1 c levels in T2 DM patients with NAFLD were significantly increased,while the high density lipoprotein cholesterol(HDL-C)was significantly decreased(all P < 0.05).2.Compared with the NAFLD group,the fasting blood glucose(FBG),hemoglobin A1 c,insulin resistance index,and high density lipoprotein cholesterol(HDL-C)in T2 DM patients with NAFLD were significantly increased(P<0.05).Compared with T2 DM group,urea nitrogen and TG in T2 DM combined with NAFLD group were significantly higher than those in T2 DM group(P<0.05),and the levels of FABP4,Hb A1 c,insulin resistance index and fasting blood glucose were higher than those in T2 DM group.3.FABP4 level was positively correlated with body weight,abdominal circumference,FINS and insulin resistance index(P<0.05).4.Elevated levels of FABP4,body weight,abdominal circumference,BMI,ALT,TG,Hb A1 c,HOMA-IR are associated risk factors for type 2 diabetes with non-alcoholic fatty liver disease.5.The ability of FABP4 to distinguish between disease state and control group was assessed by ROC analysis.Area under the curve(95% confidence interval)=0.905(0.852-0.958,P<0.05).Conclusions:(1)The levels of body weight,abdominal circumference,BMI,fasting blood glucose,insulin resistance index and Hb A1 c in T2 DM patients with NAFLD were significantly increased.(2)There was a significant positive correlation between serum FABP4 level and body weight,abdominal circumference,FINS and insulin resistance index.(3)Serum FABP4 levels are elevated in patients with NAFLD and T2 DM,and are further elevated when both diseases are present.Serum FABP4 concentration is a potential diagnostic biomarker for predicting NAFLD and T2 DM,and may predict deterioration of metabolic status.(4)Increased levels of FABP4,body weight,abdominal circumference,BMI,TG,Hb A1 c and HOMA-IR are risk factors for type 2diabetes and non-alcoholic fatty liver disease. |
