Clinical Observation Of Guan Acupuncture Therapy Based On The Tendon Theory In Treatment Of Knee Osteoarthritis

Objective: Observe the clinical efficacy of Guan acupuncture therapy based on the tendon theory in treatment of knee osteoarthritis,providing clinical experiment support for further treatment of knee osteoarthritis.Methods: Eighty patients with Zuyangming tendon type knee osteoarthritis who met the inclusion criteria were selected from the outpatients of Department of acupuncture and moxibustion,Shijiazhuang Hospital of Traditional Chinese medicine from January 2022 to November2022.Patients were randomly divided into treatment group(n=40)and control group(n=40).Basic intervention was provided for the patients from both groups such as health education,exercise and life guidance.And the treatment group was treated with Guan acupuncture therapy based on the tendon theory(acupoints selection: Hedingci,Binneishang,Binneixia,Binwaishang,Binwaixia,Tibial tubercle,Binxia,Zusanlici),while the control group was treated with common acupuncture(acupoints selection:Neixiyan,Waixiyan,Xuehai,Yanglingquan,Yinlingquan,Liangqiu,Zusanli).The needle retaining time of the two groups were 30 minutes,once every other day,the patients were treated continuously for 4 weeks.Clinical efficacy was evaluated by comparing the visual analogue scale(VAS),the Western Ontario and Mc Master University Osteoarthritis Index(WOMAC)scores,and knee range of montion after the end of treatment,while the final results were presented by combining the WOMAC scores.Shedding and adverse events were also recorded at the same time.Finally,SPSS 25.0 was used to analyze the datas and evaluate the efficacy.Results: Statistically difference was not found in the sex,age,disease duration,disease location(affected knee),VAS scores,WOMAC scores,and knee range of motion(P>0.05),and the datas were comparable.(1)VAS scores: Comparison between groups,the VAS scores of the treatment group(Guan acupuncture based on tendon theory)were lower than the control group(common acupuncture)after treatment,and there was a statistically difference(P<0.05);and at time of follow-up 1 month,the VAS scores of treatment group(Guan acupuncture based on tendon theory)were significant lower than the control group(common acupuncture),and there was a significant statistically difference(P<0.01).Comparisons within group,the VAS scores of both groups after treatment and follow-up 1 month were lower than before teratment,and there was a significant statistically difference(P<0.01);while comparing the VAS scores of both groups after treatment and follow-up 1 month,there was a significant statistically difference(P<0.01),too.(2)WOMAC scores: Comparison between groups,the WOMAC pain scores,WOMAC stiffness symptom scores,WOMAC daily activity function scores,WOMAC total scores of the treatment group(Guan acupuncture based on tendon theory)in both groups were lower than the control group(common acupuncture)after treatment and follow-up 1month,and there was a significant statistically difference(P<0.01).Comparisons within group,the WOMAC pain scores,WOMAC stiffness symptom scores,WOMAC daily activity function scores,WOMAC total scores in both groups after treatment and follow-up 1month were lower than before teratment,and there was a significant statistically difference(P<0.01);while comparing the fore-mentioned scores of both groups after treatment and follow-up 1 month,there was a significant statistically difference(P<0.01),too.(3)Knee range of motion: Comparison between groups,knee range of motion in the treatment group(Guan acupuncture based on tendon theory)were higher than the control group(common acupuncture)after treatment and follow-up 1 month,and there was a statistically difference(P<0.05).Comparisons within group,knee range of motion in both groups after treatment and follow-up 1month were higher than before teratment,and there was a significant statistically difference(P<0.01);while comparing knee range of motion in both groups after treatment and follow-up 1 month,there was a significant statistically difference(P<0.01),too.(4)Clinical efficacy: The total effective rate of the treatment group(Guan acupuncture based on tendon theory)was 94.74%,and the total effective rate of the control group(common acupuncture)was 78.38%.There was a statistically difference between the groups(P<0.05).No adverse events were found in this study,and all subjects had stable vital signs without any discomfort symptoms.Conclusions: Both of the common acupuncture therapy and the Guan acupuncture therapy based on tendon theory had clinical effect on the Zuyangming type of KOA,in contrast,clinical efficacy of the treatment group was more significant and lasting.The Guan acupuncture therapy based on the tendon theory in treatment of knee osteoarthritis was reliable,and it can effectively improve the pain and stiffness symptoms of KOA patients.At the same time,it can effectively correct the "Imbalance between bones and muscles",and improve the mobility as well as the joint function of knee.
