Purpose: The goal of the experimental study is to observe the clinical efficacy of the tendons theoretical guidance acupuncture treatment for knee osteoarthritis(KOA).Material and method: The first section of thesis is the literature research and summary review of traditional medicine tendons theory on the understanding and treatment of knee osteoarthritis. 60 KOA patients’ records met the inclusion criteriawere selected in the clinical study, randomly divided into two groups – conventional acupuncture group and tendons acupuncture group, and compared the treatments of two groups. The treatment of conventional acupuncture group used the methods in a teaching material named ‘Acupuncture’ by Xuemin Shi Fifth Edition as reference. By the tendons theory, the method chose 4 acupuncture points –Neixiyan,Dubi,Yinlingquan,Yanglingquan,acupuncture treatment for half an hour, and then treated for 15 days by normal treats on the conventional acupuncture depth. The treatment of tendons acupuncture group utilized tendons theory as the guiding principle, also the principle of ‘serch points based on pain’, selected 4 focus points to do the acupuncture treatment. The treatment time, methods and treatment days of two groups were identical. The study also estimated the VAS and WOMAC tables before and after the treatments for both groups, and finished the statistical analysis of the test data. After 3 months, called the patients for further check and feedbacks to get returning data.Results: The analysis of patients’ basic information 60 KOA patients selected by this thesis were all from Affiliated Hospital of Liaoning University of Traditional Chinese Medicine, Department of bone K. The efficacy of themwere discontinued. The rest 60 patients were randomly divided into conventional acupuncture group and tendons acupuncture group. The study did the statistical analysis on the test data for gender, age, disease position, and sick time. The p-value results were all greater than 0.05, and there’s no significant difference. This illustrates that two sets of data were comparable. 2. The analysis of the assessments of patients’ sick condition before the treatments The study assessed the patients’ sick condition by using VAS and WOMAC tables before the treatments. Statistical analysis was performed on the scores of VAS, WOMAC pain, WOMAC stiff, and WOMAC daily activities between two groups. The p-value results were all greater than 0.05, which meant there’s no significant difference. Therefore, two sets of data were comparable. 3. The analysis of the assessments of patients’ sick condition after the treatments The study assessed the patients’ sick condition by using VAS and WOMAC tables after the treatments. Statistical analysis was performed on the scores of VAS, WOMAC pain, WOMAC stiff, and WOMAC daily activities comparing the before and after scores of the treatments in each group internally. The p-value results were all less than 0.01 which illustrates that there was a significant difference between the before and after the treatments. In other words, both conventional acupuncture and tendons acupuncture treatments had good clinical efficacies. Comparing with the after treatment score data of two groups, the p-value results were greater than 0.05 which was not significant. And the ratio of effective data of both groups were 93.3%. This demonstrates that the clinical efficacy of the conventional acupuncture and tendons acupuncture had no clear distinction. 4. The analysis of the returning data The study collected the data again after 3 months for both conventional acupuncture and tendons acupuncture groups. 3 patients relapsed in both groups. The ratio of the relapse was 10%. The two sets of data are identical which illustrates that the relapse effects of the conventional acupuncture and tendons acupuncture were approximately the same.Conclusion: 1. Significant effect Tendons acupuncture method is the treatment under the guidance of the tendons theory. It hasa significant effect on the treatment for KOA patients. Comparing with the efficacy of Traditional meridian theory of acupuncture acupoints by statistical analysis, there’s no significant difference. The clinical efficacies are identical. 2.Dialectical simplicity Conventional methods of traditional acupuncture need to do the dialectical analysis first on patients. The process of the dialectical analysis is really complex. Doctors might have wrong dialectical analysis which will influence the treatment effects. However, the dialectics of the tendons theory is simple and faultless. It discriminates the tendons positions based on the focus points. The treatment effects of is similar to conventional acupuncture. 3.Fetching acupuncture points conveniently After the doctors analyzing the dialectics on patients by traditional acupuncture meridians, fetch the patients acupuncture points based on the dialectical types. Meridian acupoint selection,local acupoint selection and specific acupoints, there’re huge differences between treatment capabilities and clinical experiences of the doctors on fetching the acupuncture points. The treatment effects are obviously different. However, the tendons acupuncture method only fetches the acupuncture points from the pain focus points. It is more convenient. And the thesis study have demonstrated the clinical efficacy are similar to the traditional acupuncture meridians. 4.The breaking point of integrative medicine study There are still some doubts on the truth of traditional meridian theory, since its modern medicine specific therapeutic mechanism is not very clear. The basis of tendons theory originates from 《Huang Di Nei Jing》. With the knowledge and study of anatomy, biomechanics, and molecular biology, contemporary physicians enrich and standardize the traditional meridian theory, make it more significant in the study of integrative medicine and easier to understand and to promote. |
