Objective(s):In this study,we investigated the association between MHR and type 2 diabetic retinopathy by comparing the levels of MHR in each clinical stage of T2 DM patients with retinopathy,understanding the value of MHR for predicting,diagnosing,and assessing disease severity,Provide consultation support for early diagnosis and treatment of patients.Method(s):A total of 428 T2 DM with or without retinopathy who were hospitalized in affiliated hospital of Guilin Medical College from January 2020 to October 2022 were selected for the study,and the patients were divided into a T2 DM group based on their fundus findings according to the international clinical grading of diabetic retinopathy published in 2019 NPDR group and PDR group.The general information and laboratory data of the study subjects were collected from the discharge medical records,and the values of BMI,MHR,NLR,and NMP at presentation were calculated,collated,and analyzed using statistical methods,The differences in MHR levels among the subgroups were compared,resulting in a correlation analysis between MHR and DR.Result(s):1.MONO,MHR,NMP,NLR,UA,Scr,BUN,UACR are increased in the NPDR and PDR groups compared with the T2 DM group,while HDL-C levels are decreased,is considered statistically significant(P<0.05).2.Three groups of study subjects are regrouped,the T2 DM group as a group and the NPDR and PDR groups as DR group,and the results of univariate analysis show that MONO,HDL-C,MHR,NMP,NLR,UA,Scr,BUN,UACR are significantly different between the two groups of T2 DM and DR(P<0.05).The results of logistic regression analysis suggested that MHR,Scr,UACR are risk factors for concurrent retinopathy in T2 DM patients(OR=5.169;95%CI:1.132-23.60;P<0.05),(OR=1.016;95%CI:1.003-1.030;P<0.05),(OR=1.002;95%CI:1.001-1.004;P<0.05).3.Correlation analysis revealed that MHR is positively correlated with NMP,TG,UA,Scr,BUN,UACR,and the severity of DR,P<0.05.Partial correlation analysis adjusted for confounding factors(gender,age,course of DM,DN,PDN,macroangiopathy,hypertension,BMI,glucose,lipid,renal function),the severity of DR is still positively correlated with MHR.4.The ROC curve for MHR prediction of type 2 diabetic retinopathy is calculated to yield an area under the curve of 0.715(95%CI:0.666-0.764,P<0.001)and the optimal cut-off value is0.498 according to the Youden index,which means that retinopathy will occur in patients with T2 DM when the MHR>0.498,with a sensitivity of 69.9% and specificity of 65.1%.Conclusion(s):1.MHR is closely associated with retinopathy in patients with T2 DM,and the expression level of MHR is positively related to the severity of type 2 diabetic retinopathy;2.MHR has a certain predictive value for T2 DM with retinopathy,When MHR>0.498,the probability of T2 DM patients with retinopathy increases.
