| Objective:By observing the changes of dermoscopic image features,total skin lesion scores,TCM symptom scores and quality of life scores of gastrointestinal damp-heat syndrome acne before and after treatment.To evaluate the clinical efficacy of self-made Sangpi Zhiqiao prescription and doxycycline hyclate enteric-coated capsules in the treatment of gastrointestinal damp-heat syndrome acne from an objective point of view.In order to develop new ideas for the treatment of acne vulgaris by the combination of traditional Chinese and western medicine.Methods:A total of 102 patients with acne vulgaris who were admitted to the dermatology department of Central Hospital of Gansu from May 2022 to January 2023 were included.General data of the subjects were recorded at the initial visit.102 subjects were randomly divided into two groups.The control group(51 cases)was given doxycycline hyclate enteric-coated capsules orally,combined with nafloxacin cream and benzoyl peroxide gel for topical use.The treatment group(51 cases)was given self-made Sangpi Zhiqiao prescription,doxycycline hyclate enteric-coated capsules,nafloxacin cream and benzoyl peroxide gel.The treatment continued for four weeks.The bilateral buccal target lesions’ images of background color,blood vessel morphology,blood vessel distribution,pustules,hair follicle horn plugs,perifollicular light yellow halo and scales under dermoscopy were observed and evaluated respectively before treatment,after two weeks of treatment and after four weeks after treatment.The changes of total skin lesion scores and TCM symptom scores of patients were recorded,and the life quality score of patients was collected before treatment and four weeks after treatment,and the adverse reactions of patients in the course of treatment were closely monitored.Finally,through the statistical and comparative analysis of the test results,the conclusions were drawn.Results:1.A total of 102 patients were included in this study,9 cases were excluded,and a total of 93 patients completed the clinical trial,46 cases in the treatment group and 47 cases in the control group.Compared with the general data of the two groups before treatment,there were no significant differences between the treatment group and the control group in terms of gender,age and course of disease(P>0.05).2.After four weeks of treatment,the red and purplish red background color of patients decreased in both the treatment group and the control group compared with that before treatment(P<0.05).The red and purplish red background of the treatment group was relatively less than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the light red background increased significantly,and the difference was statistically significant(P<0.05).Pustules,hair follicle horn plugs,perifollicular light yellow halo and scales were reduced in two groups compared with before treatment,and the difference was statistically significant(P<0.05).Compared with the control group,the scales in the treatment group were significantly reduced,and the difference was statistically significant(P<0.05).There were no significant differences in the changes of yellowish red background,blood vessel morphology and blood vessel distribution between two groups before and after treatment(P>0.05).3.After four weeks of treatment,the total effective rate of the self-made Sangpi Zhiqiao prescription group was 87.00%.The control group’s total effective rate was 68.10%,the difference between the two groups was statistically significant(P<0.05).The total skin lesion scores and TCM symptom scores of two groups were decreased compared with before treatment,and the difference was statistically significant(P<0.05).Patients’ quality of life also improved.The results indicated that self-made Sangpi Zhiqiao prescription combined with doxycycline hyclate enteric-coated capsules,benzoyl peroxide gel and nafloxacin cream had better clinical effect on gastrointestinal damp-heat syndrome acne than western medicine therapy alone.4.A total of three cases of mild adverse reactions occurred in both groups,including 2cases in the treatment group and 1 case in the control group.Patients were instructed to apply benzoyl peroxide gel on the local spot accurately and to combine hyaluronic acid gel.All the symptoms were improved without affecting subsequent treatment.Conclusion:1.The background color,pustules,hair follicle horn plugs,perifollicular light yellow halo and scales of bilateral buccal target lesions in patients with gastrointestinal damp-heat syndrome acne were changed under dermoscopy before and after treatment,while the blood vessel morphology and blood vessel distribution were not significantly changed.2.Self-made Sangpi Zhiqiao prescription combined with doxycycline hyclate enteric-coated capsules orally,benzoyl peroxide gel and nafloxacin cream in the treatment of gastrointestinal damp-heat syndrome acne.The patients’ skin lesions,TCM symptoms and quality of life were all improved,which is worthy of clinical application. |
