| Objective: Observe the efficacy and safety of Ganjiang Fuzi Decoction with Wupi Decoction in the treatment of Yang deficiency and water accumulation type of chronic heart failure patients through a randomized controlled clinical trial, providing reference data and clinical ideas for the treatment of chronic heart failure with traditional Chinese medicine. Methods: Recruiting 66 patients with Yang deficiency and water accumulation chronic heart failure who attended the outpatient and inpatient Departments of Cardiology,Inner Mongolia Hospital of Traditional Chinese Medicine between January 2022 and September 2022,and they were randomly divided into treatment and control groups, with 33 cases in each group. The control group was treated with conventional western medicine, and the treatment group was treated add Ganjiang Fuzi Decoction with Wupi Decoction for a period of 4 weeks.Changes in left ventricular ejection fraction(LVEF),N-terminal B-type natriuretic peptide(NT-pro BNP),NYHA cardiac function classification,TCM symptom score,6-minute walk distance, and the Kansas City Quality of Survival Scale for patients were observed before and after treatment in both groups, and safety was evaluated according to routine blood test, liver and kidney function, ions, electrocardiogram. Results: A total of 66 patients with Yang deficiency and water accumulation chronic heart failure were included in this study,33 cases in each group. The final trial was completed in 61 cases,of which 31 cases were completed in the treatment group and 30 cases in the control group.1.Baseline data: Comparison of age, gender, disease duration,NYHA cardiac function classification, heart rate, systolic and diastolic blood pressure, primary disease between the two groups was not statistically significant(P>0.05)and was comparable between groups.2.Left ventricular ejection fraction: The difference in left ventricular ejection fraction between the two groups was not statistically significant before and after treatment(P>0.05),nor was the difference between the treatment and control groups(P>0.05).3.NT-pro BNP: NT-pro BNP decreased in both groups after treatment compared to before treatment(P<0.05),and the decrease was more significant in the treatment group(P<0.05).4.NYHA cardiac function efficacy: After treatment, the effective rate was 64.52% in the treatment group and 53.33% in the control group, with no statistically significant difference(P>0.05).5.TCM syndrome score: The TCM symptom score decreased in both groups after treatment compared to before(P<0.05),and the decrease was more significant in the treatment group(P<0.05).6.6-minute walking distance :The 6-minute walking distance was longer in both groups after treatment compared to before(P<0.05)and was more significantly longer in the treatment group(P<0.05).7.Kansas City Cardiomyopathy Patients’ Quality of Life Scale: The Kansas City Cardiomyopathy Patients’ Quality of Life Scale score improved in both groups after treatment compared to before(P<0.05),and the improvement was more significant in the treatment group(P<0.05).Conclusion: Patients with Yang deficiency and water accumulation chronic heart failure showed further reduction in NT-pro BNP level, improvement in walking distance and quality of life with no significant adverse effects after the addition of Ganjiang Fuzi Decoction with Wupi Decoction on top of conventional western medicine treatment. |
