Clinical Observation Of Modified Linggui Zhugan Decoction In The Treatment Of HFmrEF Yang Deficiency Water Flooding Syndrome

Objective In this study,Linggui Zhugan Decoction was used to treat patients with chronic heart failure with mild reduction of ejection fraction and Yang deficiency and water flooding syndrome,to observe its clinical efficacy,to explore the possible mechanism of action,and to provide new ideas for the treatment of chronic heart failure with integrated traditional Chinese and Western medicine.Method From October 2021 to December 2022,patients with chronic heart failure with Yang deficiency and water flooding syndrome who met the inclusion criteria in Yunnan Provincial Hospital of Traditional Chinese Medicine were randomly divided into treatment group and control group.The control group was treated with conventional western medicine.On this basis,the treatment group was supplemented with Ling gui Zhugan Decoction for oral administration.The two groups were compared after14 days of treatment.The changes of NT-pro BNP,cardiac color ultrasound(LVEF,LAD,LVEDD,CO,CI)before and after treatment and the changes of NYHA cardiac function classification,6-minute walking test distance and TCM symptom score were analyzed by SPSS26.0 software to evaluate its clinical efficacy.Result(1)Pre-treatment data:There was no statistical difference in baseline data between the two groups.(2)Comparison of the efficacy of TCM syndromes:After treatment,the total effective rate of TCM syndromes was 87.5% in the treatment group and 70% in the control group.The treatment group was significantly better than the control group in impr-oving the efficacy of TCM syndromes(P<0.01).The scores of palpitation,asthma,limb edema,fatigue,expectoration and oliguria in the single symptom score of traditional Chinese medicine were statistically different between the two groups.Although the scores of cyanosis,chills and cold limbs and abdominal distension were improved after treatment,there was no statistical difference between the two groups.(3)N-terminal B-type natriuretic peptide:After treatment,the level of NT-proBNP in the two groups decreased significantly,and the treatment group was better than the control group(P<0.05).(4)Cardiac color ultrasound examination indexes:After treatment,the levels of cardiac color ultrasound examination indexes in the two groups were improved,and the difference was statistically significant(P<0.01).The treatment group was more effective in improving LVEF,LAD,LVEDD,CO and CI.(5)Comparison of NYHA cardiac function classification:NYHA cardiac function class ification was improved in both groups after treatment,and the total effective rate of the treatment group was 78.12%.The curative effect of the treatmentgroup was better than that of the control group(P<0.05).(6)6-minute walking test distance:After treatment,the 6MWT of both groups improved,and the performance of the treatment group was better than that of the control group(P<0.05).(7)Safety narrative:During the course of this study,the vital signs of the two groups of patients were stable,and the safety indicators of the two groups of patients before and after treatment were not abnormal during the treatment period,and no adverse reactions occurred during the medication period.Conclusion Linggui Zhugan Decoction combined with conventional western medicine basic treatment of chronic heart failure with mild reduction of ejection fraction has a good effect on patients with Yang deficiency and water flooding syndrome.It can improve the clinical symptoms of patients,and has high safety.It can provide some reference value for the future clinical treatment of chronic heart failure with integrated traditional Chinese and western medicine.
