| Objective To investigate the relationship between the level of GDNF,s Nf L and NLRP3 in serum and depression in patients with acute ischemic stroke after clinical treatment.Methods A total of 82 patients with acute ischemic stroke were collected and followed up(hospitalized in the Department of Neurology,the First Affiliated Hospital of Anhui University of Science and Technology from August 2021 to May 2022,and followed up in the outpatient department from September 2021 to November 2022).The serum levels of GDNF,s Nf L and NLRP3 were detected by ELISA and followed up in the clinic after discharge.Using the Hamilton Depression Scale assessment,those who scored more than 7 were included in the depressed group,and the rest were the non-depressed group.The serum levels of GDNF,s Nf L and NLRP3 in the depressed group and the non-depressed group were compared,univariate and multivariate analysis(binary logistic regression analysis)was performed,and ROC curves were plotted.Results The study on the score of patients who completed follow-up found that the NIHSS score in the post-stroke depression(PSD)group was higher than that in the non-PSD group at follow-up[PSD:6 points,non-PSD: 5 points,P=0.011].The HAMD score in PSD group was higher than that in non-PSD group [PSD:10 points,non-PSD :4 points,P< 0.05].The NIHSS in the PSD group was slightly higher at follow-up than at hospitalization.The incidence of patients discharged more than3 months was 17.07%,while the incidence of patients discharged less than 3 months was 13.41%.Serum GDNF level in PSD group was 3.81pg/m L,which was lower than that in non-PSD group[(6.32pg/m L),z=-5.898,P<0.05].The level of NLRP3 inflammasome in PSD group was24.41pg/m L,which was higher than that in non-PSD group [(19.75pg/m L),z=-3.359,P<0.05].The level of serum Nf L in PSD group was 592.46pg/m L,higher than that in non-PSD group[(512.76pg/m L),z=-3.525,P<0.05].spearman Spearman test was performed after serum GDNF,NLRP3 inflammassome and s Nf L levels were skewed,and the results showed that NLRP3 level in PSD group was positively correlated with HAMD score at follow-up(r=0.424,P<0.05).Univariate logistic regression analysis showed that low levels of serum GDNF had a higher risk of depression,and the two were negatively correlated(OR=0.225,95%CI: 0.115-0.440,P<0.05).High levels of serum NLRP3 inflammasome were associated with a higher risk of depression(OR =1.319,95%CI:1.138-1.529,P<0.05).High levels of serum Nf L were associated with a higher risk of depression(OR =1.009,95%CI: 1.003~1.016,P=0.003).Multivariate analysis: Only serum GDNF and NLRP3 inflammasome were statistically significant.ROC curve: The sensitivity of GDNF and NLRP3 inflammasome detection was significantly higher than that of NLRP3 alone(0.88),and the specificity was also better.Conclusion The levels of NLRP3 inflammasome and s Nf L in the patients with post-stroke depression were significantly higher than those in the non-depressed group,while the levels of GDNF were significantly lower than those in the non-depressed group.The results suggest that abnormal serum GDNF and NLRP3 levels in patients with acute stroke are risk factors for later depressive mood in patients,and the combined detection of the two can improve the sensitivity of diagnosing PSD and can simply predict whether patients will develop PSD in clinical practice,so as to provide certain references for clinical diagnosis.Figure 3 Table 8 Reference 90... |
