| In recent years,with the continuous improvement of the national economic level,people pay more attention to the quality of medical care.The country has developed from a planned economy to a market economy,encouraged the public to run hospitals,relaxed the policy,promoted the cooperation between the government and private capital in the field of health,and gradually reduced the economic investment in public hospitals.In this context,the traditional way of calculating medical staff performance commissions through the balance method leads to the deviation of public hospitals from the nature of public welfare and the pursuit of interests,leading to the exacerbation of contradictions between doctors and patients.Aiming at the reform of public hospitals,the state has introduced a large number of new policies to improve medical and health quality,forcing public hospitals to break the profitseeking mechanism and return to the "public welfare" nature of medical treatment in public hospitals.New politics,new medical reform and new situation prompt public hospitals to update and iterate performance management.Through continuous exploration by public hospitals and combined with national policy guidance,various advanced methods of performance evaluation have been introduced.The balanced scorecard performance evaluation method to improve the sustainable development of public hospitals and maintain their public welfare is more and more recognized.This paper studies the construction of balanced scorecard performance evaluation system of Z public hospital by case analysis and expert consultation.First of all,the reform process of public hospitals,the concept of performance evaluation,the concept of balanced scorecard,the concept of disease diagnosis related groups,the application of balanced scorecard in public hospitals and incentive theory are described in detail to provide theoretical support for the following paper.Secondly,taking Z public Hospital as a case,after field understanding and obtaining relevant data,summarizing its basic situation and current performance evaluation index system,analyzing its existing problems and causes,such as low compatibility between performance evaluation index system and strategic objectives,unscientific dimension setting,no separate financial dimension setting,inadequate selection of evaluation indexes,etc.It points out the feasibility and necessity of constructing balanced scorecard.Then,based on the strategic objectives,environment and social mission of Z public hospital and the current situation of the hospital,the performance evaluation system of financial dimension,customer dimension,internal operation dimension and learning and growth dimension is constructed based on the theoretical logic of balanced scorecard.The following are selected: Evaluation indexes such as the proportion of outpatient income in medical income,the proportion of inpatient income in medical income,the complexity of disease(CMI),the inclusion rate of DRG,the increase of per capita outpatient cost and the proportion of senior professional title personnel,etc.Based on the rebuilt performance appraisal index system,this paper compares and analyzes the results obtained from the two-dimensional performance appraisal system based on DRG and resource-consumption-based RBRVS that Z public hospital formally introduced and implemented in 2020,and proposes the following ways to improve performance: First,within the reasonable scope of the policies under the new medical reform and the relevant regulations of the state on public hospitals,the purpose of reducing the burden of patients and improving patients’ trust in hospitals;Second,strengthen the financial management and optimize the cost management process,so that the hospital can balance its income and expenditure to ensure its long-term operation,and at the same time have the ability to continue to maintain its public welfare,safeguard social public health security,and protect people’s health social responsibility;Third,strengthen the application and assessment of DRG and CMI,promote the reform of grouping payment related to disease diagnosis,help improve the quality of medical services,provide patients with better medical experience,and provide certain references for the construction of the performance evaluation index system of public hospitals. |
