Data Mining Based Analysis Of Professor Ruan Shiwei’s Medication Pattern And Academic Experience In The Treatment Of Chronic Kidney Disease With Kidney Deficiency And Damp Heat

Objective:Through the data mining method,Professor Ruan Shiwei’s medication rules in the treatment of chronic kidney disease with kidney deficiency and damp heat were extracted,and then the academic experience of the instructor in the diagnosis and treatment of this disease was summarized,so as to provide guidance and reference for the treatment of chronic kidney disease with kidney deficiency and damp heat.Methods:The case data of patients with chronic kidney disease with kidney deficiency and damp heat who attended the clinic of Professor Ruan Shiwei from January 2019 to January 2022,famous doctor in the Affiliated People’s Hospital of Fujian University of Traditional Chinese Medicine,met the inclusion criteria of this study were comprehensively collected,organized,and entered into the "Traditional Chinese Medicine Inheritance Support Platform(V2.5)",and use modules such as frequency statistics,cluster analysis,and association rule method to mine potential interrelationships between the data.Results:1.A total of 180 patients and 610 prescriptions were included in this study.Among them,there are more women than men,and the age distribution is mainly concentrated in the age range of 40 to 69 years old.In terms of staging,there were 52 cases of chronic kidney disease stage 1,63 cases of stage 2,47 cases of stage 3,14 cases of stage 4,and 4 cases of stage 5.Chronic glomerulonephritis accounted for the highest proportion of 30.56% of primary diseases with clear etiology,followed by Ig A nephropathy at 20.00%.According to statistics,the common symptoms of patients are fatigue,dry mouth,soreness of waist and knees,bitter mouth,increased nocturia,yellow and red urine,lack of food,foamy urine,edema,and sticky mouth.Common tongue textures include light red tongue,red tongue,dark red tongue,and tooth-marked tongue.Yellow greasy fur,thin yellow fur,white greasy fur,and thin white fur are common tongue fur.Common pulse conditions are slippery,thin,deep,and slippery pulse.2.There are 69 traditional Chinese medicines that have been used more than 30 times,including rehmannia glutinosa,plantago seed,astragalus root,anemarrhena anemarrhena,phellodendron phellodendri,licorice,fructus nelumbinis,ophiopogon japonicus,talc,smilax,liuyuexue,etc.In terms of medicinal properties,cold,mild,and warm medicinals are the majority,and cold medicinals account for the highest proportion at 53.36%.In terms of the taste of medicines,sweet,bitter and pungent medicines are mostly,of which sweet medicines accounted for the highest proportion,which was 41.02%.In terms of meridians,drugs are mostly attributed to the kidney,spleen,lung,liver,heart,and bladder meridians,among which the kidney meridian accounts for the highest proportion,accounting for 20.41%.3.Through the analysis of prescription rules,37 groups of medication patterns were obtained.After review by Professor Ruan,some commonly used drug pairs were selected as follows:(1)Anemarrhena,raw rehmannia glutinosa;(2)Talc,bamboo leaves;(3)Astragalus,fructus nelumbinis.Commonly used horn medicines and medicinal strings:(1)Plantago Seed,raw rehmannia glutinosa,Watermelon peel;(2)Plantago Seed,Tuckahoe,Moutan Bark;(3)Cornus officinalis,Tuckahoe,raw rehmannia glutinosa,Moutan Bark.Through the analysis of prescription rules,16 drug combinations were extracted,and after eliminating repeated combinations,the commonly used combinations are as follows:(1)raw rehmannia glutinosa,watermelon peel,and plantain seed;(2)Tuckahoe,raw rehmannia glutinosa,Moutan Bark,Cornus officinalis;(3)Anemarrhena,Tuckahoe,raw rehmannia glutinosa;(4)Plantago Seed,Moutan Bark,Tuckahoe.4.Through the mutual information method of new prescription analysis,20 new drug combinations were obtained,which are:(1)bamboo leaves,astragalus,talc;(2)watermelon peel,Astragalus,plantain grass;(3)Capillaris wormwood herb,Divaricate Saposhniovia Root,Notopterygium;(4)Rehmannia glutinosa,Asparagus cochinchinensis,Polygonum cuspidatum;(5)Purslane,Eupatorii Herba,Feather Cockscomb Seed;(6)Raw rehmannia glutinosa,Atractlodis Rhizoma,Junesnow;(7)Raw rehmannia glutinosa,dogwood,licorice;(8)Sophora flavescens,shepherd’s purse,rhubarb;(9)Prunella vulgaris,Radix Glehniae,Toosendan;(10)Forsythia,almond,mulberry bark,red bean,ephedra;(11)Ophiopogon japonicus,astragalus,and Lycii Cortex;(12)Watermelon peel,psyllium,Moutan bark,ginger Pinellia;(13)Angelicae Pubescentis Radix,Divaricate Saposhniovia Root,Notopterygium;(14)Rehmannia glutinosa,Asparagus cochinchinensis,Eucommia;(15)Purslane,Eupatorii Herba,Solanum nigrum;(16)Atractlodis Rhizoma,Kochia chinensis,June snow;(17)Dogwood,licorice,Tuckahoe,Moutan bark;(18)Shepherd’s purse,Semen Astragall Complanati,rhubarb;(19)Uncaria,Amomum,Prunella vulgaris,Radix Glehniae;(20)almonds,red beans,loquat leaves,Blackberrylily Rhizome,Radix Curcumae.5.Through entropy clustering analysis,10 new prescription combinations can be extracted,namely,(1)Bamboo leaves,Astragalus,Talc,Ophiopogon japonicus,Lycii Cortex.(2)Watermelon peel,plantain seeds,Moutan bark,Jiang Pinellia.(3)Capillaris wormwood herb,Divaricate Saposhniovia Root,Notopterygium,Angelicae Pubescentis Radix.(4)Rehmannia glutinosa,Asparagus cochinchinensis,Polygonum cuspidatum,Eucommia ulmoides.(5)Purslane,Eupatorii Herba,Feather Cockscomb Seed,Solanum nigrum;(6)Rehmannia glutinosa,herb,Junesnow,Kochia chinensis.(7)Raw rehmannia glutinosa,dogwood,licorice,Smilax tuckahoe,Moutan bark.(8)Sophora flavescens,shepherd’s purse,rhubarb,Semen Astragall Complanati.(9)Prunella vulgaris,Radix Glehniae,,Toosendan,Uncaria,Amomum.(10)Forsythia,almonds,Morus alba,Chixiaodou,Ephedra,Loquat leaves,Blackberrylily Rhizome,Radix Curcumae.Conclusion:1.Chronic kidney disease is a disease characterized by deficiency of the essence and deficiency of the substance.The syndrome of kidney deficiency and dampness-heat can be distributed throughout the course of chronic kidney disease,mainly concentrated in the early and middle stages of the disease.2.The more commonly used drugs used by Professor Ruan Shiwei in the treatment of chronic kidney disease with kidney deficiency and damp-heat syndrome include: Raw rehmannia glutinosa,Plantago seed,Anemarrhena,Cortex Phellodendron,Licorice,fructus nelumbinis,talc,Ophiopogon japonicus,Smilax tuckahoe,June snow,bamboo leaves,etc.The selection of medicines mostly starts with heat-clearing,diuretic and damp-draining,invigorating qi,nourishing yin,relieving exterior syndrome.3.In terms of prescription,Radix dihuang Decoction formula,Qingxin lotus seed drink,Li’s or Wang’s decoction to clear heat and Yiqi,Zhongjiao Xuanbi decoction,ephedrine forsythiae red bean soup and so on are commonly used as the core prescription,according to the combination of pathogenesis,disease location and disease tendency.4.Professor Ruan Shiwei combined with three factors to grasp the basic pathogenesis of diseases,pay attention to the differentiation of Zang Fu organs and syndrome to distinguish the disease location,syndrome differentiation of good and evil to distinguish the disease,benefit the kidney and restore the normal,clear the interest to remove the evil,treat both symptoms and root causes.
