| As a national medical master,Professor Lv Renhe has long been engaged in the clinical treatment and academic research of chronic kidney disease,diabetes mellitus and other internal diseases.In his clinical practice,Prof.Lv emphasizes on "inheriting the ancient and seeking the good",and is good at learning from the ancient and modern medical treatises and diligently studying the research results and methods of modern medicine.Prof.Lv has been engaged in front-line clinical work for many years,and has rich academic ideas and clinical experience,gradually formed an academic system for the treatment of chronic kidney disease during his years of clinical work and academic research,and creatively proposed academic ideas such as "miniature obstruction" and "six pairs of treatment".The clinical efficacy is remarkable.Therefore,Professor Lv’s medical growth and experience are valuable assets for our descendants,and it is of great academic significance and clinical value to summarize and study Professor Lv’s academic experience in the treatment of chronic kidney disease.This project is based on the 2018 National Key Research and Development Program led by Beijing University of Chinese Medicine,"Research on the inheritance and innovation of famous old Chinese medicine experience based on the idea of ’combination of Taoism and art’and multi-fusion method".The research method of rooted theory and data mining was used to analyze the interviews and medical cases of famous veteran Chinese medicine practitioners,to summarize the academic thought and treatment experience of Professor Lv Renhe in treating chronic kidney disease from both "Tao" and "Technique" aspects,to build a theoretical model of Professor Lu’s treatment of chronic kidney disease,and to explore the theoretical model of Professor Lu’s treatment of chronic kidney disease.The theoretical model of Professor Lv’s treatment of chronic kidney disease was constructed,and the rules of medicine used by Professor Lv in the treatment of chronic kidney disease were explored.It is expected to provide some reference for future research on the transmission of academic experience of famous veteran Chinese medicine practitioners.Objective:Based on the idea of multifaceted integration,this study applied the method of rooted theory to summarize the formation process and treatment characteristics of Professor Lv Renhe’s academic thinking in the treatment of chronic kidney disease;applied the method of data mining to analyze clinical cases of Professor Lv’s treatment of chronic kidney disease,summarize the rules of clinical prescription and medicine selection,and build a theoretical framework for the transmission of Professor Lv’s academic experience,so as to transmit both"Tao" and "Technique" of Professor Lv’s treatment of chronic kidney disease.Content:Interviews were conducted with Professor Lv Renhe himself,his disciples and patients,and the results of the interviews were compiled and coded together with the literature at three levels to summarize Professor Lv’s academic thoughts on the treatment of chronic kidney disease through the method of rooted theoretical research to explore the three aspects of Professor Lv’s state of mind.clinical thinking and clinical techniques.Patients who attended the outpatient clinic of the Department of Nephrology and Endocrinology,International Department of Dongzhimen Hospital of Professor Lv Renhe and met the diagnostic criteria of chronic kidney disease during the period of January 2012 to October 2021 were selected.We recorded the medical data of the patients during their outpatient visits and in the past,and compiled and collected standardized case information and treatment records of the patients.Data mining methods were used to process the case data and analyze Professor Lu’s clinical experience in treating patients with chronic kidney disease.Method:In-depth interviews with Professor Lv Renhe himself,his disciples and patients were conducted and recorded,the recordings were organized and converted into textual materials,and literature related to Professor Lv’s treatment of chronic kidney disease was collected and organized.The interview data and literature were coded at three levels:open coding,axial coding and selective coding through the rooted theory research method.The textual data were summarized through coding analysis to construct a theoretical model of Professor Lv’s treatment of chronic kidney disease and summarize Professor Lv’s academic thoughts on the treatment of chronic kidney disease.Using the platform of famous doctors’ inheritance,we constructed a clinical case queue of Professor Lv Renhe’s treatment of chronic kidney disease,and added outpatient cases,and entered information such as patients’ complaints,current medical history,subcutaneous symptoms,tongue,pulse,Chinese medicine diagnosis,Western medicine diagnosis,prescription,number of doses,medical advice and treatment history into the platform.Data mining on core drugs,drug-symptom correlation,drug correlation,etc.was conducted through the famous doctor inheritance platform,IBM SPSS Statistics 25.0 and R4.1.0 to summarize Professor Lv’s clinical experience in treating chronic kidney disease with medication.Results:Based on the rooted theory research method,we combined the "Tao" and "Technique"aspects of Professor Lv Renhe’s treatment of chronic kidney disease as the basis for improving the research on the transmission of Professor Lv Renhe’s academic thought.By coding and classifying the interview data of Prof.Lv Renhe,his disciples and patients,we explored the contents of the inheritance of Prof.Lv’s way of being a person,a doctor,a teacher and a scholar,and extracted three major categories:state of mind,clinical thinking and clinical techniques.The results of the study show that Professor Lv is ambitious.benevolent and virtuous,and his academic thinking is mainly derived from his teachers and self-study,with emphasis on the use of the past for the present and the use of the past for the present,with both inheritance and innovation.After analyzing the interrelationship between the categories.the research results show that Professor Lv’s Daoism is concentrated in four aspects,which are thought,behavior.theory and practice.Among them.Tao is more focused on thought and theory.and Art is more focused on behavior and practice.A total of 294 patients were included in this study,and the prescriptions involved 159 types of traditional Chinese medicine with a total frequency of 2304 times.The drug properties were mainly flat,cold and warm,and the drug flavors were mainly sweet and bitter,and the four meridians were mainly liver,heart,spleen and kidney.Drug-weighting level analysis showed that the core drugs were Salvia miltiorrhiza,poria,and Chuanxiong.Drug-drug co-occurrence analysis showed that the commonly used drug pairs were citron-Foji,peach kernel-red flower,and red peony-danpi;cluster analysis showed nine drug clusters,reflecting Professor Lv Renhe’s academic ideas of treating the liver and kidney together,promoting blood circulation and diuresis,attaching importance to drug-to-medicine strings and staging discriminatory treatment.Conclusion:1 Professor Lv Renhe has inherited from his wise teachers,and has closely integrated the two aspects of medical philosophy and medical art,forming the treatment principle of "using the ancient for the present and the foreign for the Chinese for the good" and the academic characteristics of "simple medicine with strange effect".2 In the clinical treatment of chronic kidney disease,Professor Lv Renhe emphasizes staged diagnosis and treatment,and applies drug pairs and drug strings for different symptoms and signs,and the core treatment method is to invigorate blood and disperse nodules,emphasizing the balance between helping the positive and eliminating the evil.3 This study is based on multiple fusion theory,applying rooting theory and data mining research methods,and the research process can provide some reference for inheriting the academic experience of famous old Chinese medicine practitioners. |
