Prevalence And Influence Factors Of Atrial Fibrillation In Hypertensive Patients In Urban And Rural Regions Of Liaoning Province

Objective:To understand the current situation and characteristics of atrial fibrillation in hypertensive patients in some area of Liaoning Province,and to clarify the status of awareness,treatment and combined risk factors of AF patients,and to summarize the main influencing factors of AF in hypertensive patients,so as to provide a reference for formulating comprehensive and effective prevention and control strategies in the future.Methods:From September 2017 to March 2019,a multi-stage stratified cluster random sampling method was used to randomly select 8876 residents ≥40 years from 23 villages or communities(9 in Dandong and 14 in Chaoyang)in Dandong city and Chaoyang city of Liaoning Province.A questionnaire survey,physical examination and laboratory test were conducted in hypertensive patients.The risk factors for atrial fibrillation were also analyzed using univariate and multi-factorial logistic regression models.Results:The prevalence of atrial fibrillation in hypertensive patients in urban and rural areas of part of Liaoning was 1.5%,and the rate of male was higher than female(2.2% vs.1.1%,P=0.005),showed an increasing trend with the age increasing(P=0.001,P=0.007).The prevalence rates of urban and rural residents were 1.8% and 1.3%,respectively,with no statistical significance,and the prevalence rates showed an increasing trend with the increase of age(P=0.002,P<0.001).The awareness and treatment rates of atrial fibrillation were 60.3% and 41.2%,respectively,and 62.9%,57.6% and 48.6%,33.3% in urban and rural areas,respectively.Women were higher than men(72.7% vs 48.6%,54.5%vs 28.6%),and the difference was statistically significant(P=0.042,P=0.030).The prevalence of AF was 2.1%,1.5%,1.1% and 1.8% in patients with normal blood pressure,stage 1 hypertension,stage 2 hypertension and stage 3 hypertension,respectively.The prevalence of overweight or obesity,diabetes and dyslipidemia were 72.1%,41.2% and45.6%,respectively.Multivariate logistic regression analysis showed that male(OR=1.85,95%CI:1.14-3.01),older age(compared with the 40-49 age group,the 70-79 age group OR=4.91,95%CI: 1.15-20.88;≥80 years old OR=7.84,95%CI: 1.60-38.40)and diabetes mellitus(OR=1.93,95%CI: 1.18-3.15)were the main risk factors for atrial fibrillation.Conclusion:The prevalence of atrial fibrillation in hypertensive patients in some areas of Liaoning Province was 1.5%,and it is higher in men than in women.The awareness and treatment rates of patients with atrial fibrillation were 60.3% and 41.2%,respectively.The prevalence of diabetes,dyslipidemia and overweight or obesity in atrial fibrillation patients were 72.1%,41.2% and 45.6%,respectively.Men,aged 70 years and above,and diabetes were the main risk factors of atrial fibrillation in hypertensive patients.
