Objective:This study aims to retrospectively analyze clinical materials of young patients with atrial fibrillation,and compared with elderly patients with atrial fibrillation,to explore and analyze the susceptibility factors and clinical characteristics of young patients with atrial fibrillation,and guide the early prevention,diagnosis and treatment of young patients with atrial fibrillation.Method:105 young patients with atrial fibrillation and 105 elderly patients with atrial fibrillation hospitalized in the Department of cardiovascular medicine of the second hospital of Jilin University from January 2016 to October 2021 were selected.Collecting relevant general data,basic pathogeny,risk factors and complications,laboratory test indexes,examination indexes and clinical treatment.Firstly,the youth group is divided into male group and female group for intra group comparison,and then the indexes of the youth group and the elderly group are compared between the two groups,using spss25.0 statistical software for statistical analysis to screen out risk factors.P < 0.05 was statistically significant.Result:1.Comparison within the youth group1.1 General data: among the 105 young patients with atrial fibrillation who met the research criteria,there were 79 males(75.2%)and 26 females(24.8%),with a male to female ratio of about 3:1.1.2 Basic pathogeny: among 105 young patients,the most common pathogeny was hypertension(27.6%).Hypertension was the most common pathogeny in male patients(31.6%),and hyperthyroidism was the most common pathogeny in female patients(23.1%).1.3 Risk factors /Complications: among 105 young patients with atrial fibrillation,9 patients had a history of diabetes(8.6%),6 patients had a family history of atrial fibrillation(5.7%),45 patients had a history of smoking(42.9%),30 patients had a history of alcohol consumption(28.6%),and 7 patients had stroke(6.7%).The risk factors of obesity,smoking and drinking in men were significantly higher than those in women(P < 0.05).1.4 Auxiliary examination: in youth patients,there were significant differences in HDL-c,free T4 and myoglobin between male and female groups(P < 0.05).There was no significant difference between other serological test results and cardiac color Doppler ultrasound results such as LVEF,LAD,mitral regurgitation area and tricuspid regurgitation area(P > 0.05).1.5 Treatment: among 105 young patients with atrial fibrillation,(1)In terms of antithrombotic treatment,the proportion of rivaroxaban,dabigatran,warfarin,aspirin,clopidogrel and heparin were 21.0%,9.5%,4.8%,47.6%,21.9% and 40.0%respectively.(2)In terms of ventricular rate control,use β receptor blockers and digitalis accounted for 66.7% and 46.7% respectively.(3)In terms of rhythm control,the use of amiodarone,propafenone and catheter ablation accounted for 18.1%,13.3%and 12.4%.There was no significant difference in treatment methods between male group and female group(P > 0.05).2.Comparison between youth group and elderly group2.1 Basic information/Risk factors/Complications: there was no significant difference between the youth group and the elderly group in the type of atrial fibrillation,smoking history,drinking history and family history(P > 0.05).There were significant differences in age,gender,diabetes,obesity,overweight and stroke between the two groups(P<0.05).2.2 Basic pathogeny: the most common pathogeny of young patients and elderly patients is hypertension.There was significant difference in hypertension,hyperthyroidism,ischemic heart disease and dilated cardiomyopathy between the two groups(P < 0.05),but there was no significant difference in heart valve disease and congenital heart disease(P > 0.05).2.3 Auxiliary examination: there were significant differences in triglyceride,apolipoprotein B,lipoprotein(a)and free fatty acid between the two groups(P < 0.05).There was no significant difference between other serological test results and cardiac color Doppler ultrasound results such as LVEF,LAD,mitral regurgitation area and tricuspid regurgitation area(P > 0.05).2.4 Treatment: the proportion of using dabigatran(9.5% vs 0.0%)and propafenone(13.3% vs 3.8%)in the youth group was significantly higher than that in the eldly group.The proportion of using rivaroxaban(21.0% vs 56.2%),clopidogrel(21.9% vs 44.8%)and dronedarone(0.0% vs 5.7%)in the youth group was significantly lower than that in the eldly group(P < 0.05).There was no significant difference in Warfarin,aspirin,heparin β receptor blockers,digitalis,amiodarone and catheter ablation(P > 0.05).Conclusion:1.Among young patients with atrial fibrillation,men are more than women.The most common pathogeny of young patients with atrial fibrillation is hypertension.2.Compared with young female patients,obesity,smoking and drinking are the risk factors of young male patients with atrial fibrillation.Compared with elderly patients,gender,obesity,hyperthyroidism and dilated cardiomyopathy are the risk factors of young patients with atrial fibrillation.Youth people with the above risk factors should be paid early attention,so as to achieve early detection,diagnosis and treatment.
