Risk Factors Analysis Of Hospitalized Patients With Atrial Fibrillation Caused By Essential Hypertension

Objective: To investigate the independent risk factors of hospitalized patients with atrial fibrillation(AF)caused by essential hypertension.Methods: The primary research object were the 2082 patients hospitalized in our hospital cardiovascular internal medicine ward from January 1,2016 to June 30,2016.The date was collected by electronic medical record query and analysised by using the retrospective case control study.Eventually,533 hospitalization EH patients were included in this study,including 74 cases with AF as group AF,459 cases without AF as non AF group.General information,clinical data,laboratory examination results,echocardiography and X-ray chest radiograph were compared between the two groups.At last,using the multiariable Logistic regression model to identify independent risk factors for AF.Results: The two groups has a most significant difference on the age,duration of hypertension,the first systolic blood pressure and pulse pressure in the hospital,NTproBNP,serum albumin,left atrial diameter(LAD),left ventricular posterior wall thickness,left ventricular mass index,Cardiothoracic ratio(P<0.01).On the red blood cells,hemoglobin,red blood cells deposited,serum creatinine,interventricular septum thickness,left ventricular ejection fraction,the difference also has a statistical significance(P<0.05).There is no statistical significance on the gender,body mass index,the peak value of the systolic/diastolic blood pressure,the first diastolic blood pressure in the hospitial,red blood cell distribution width,serum urea nitrogen,serum uric acid,total cholesterol,triglycerides,high-density lipoprotein cholesterol,lowdensity lipoprotein cholesterol,fasting blood sugar,left ventricular diameter(P>0.05).Multiariable Logistic regression analysis showed that: LAD,NT-proBNP,age and duration of hypertension are the independent risk factors for AF;the ALB is a independent protection factor for AF.When the value of LAD was 41.5,the accuracy to judge the presence of AF can reach 80.3%.Conclusion: LAD,NT-proBNP,age and duration of hypertension can be used as the independent risk factors for AF.The level of LAD is the best indicator on the clinical application value.When the value of LAD was 41.5,the accuracy to judge the presence of AF can reach 80.3%.
