Objective:To analysis the risk factors of atrial fibrillation complicated with hypertension.Methods: According to the diagnostic criteria of hypertension in the China’s prevention and control of hypertension guidelines(2010 revised version 3),956 hypertension patients were selected in the Cardiology Department of The first affiliated hospital of Dalian Medical University during the time from January 2013 to May 2015 as research objects.The hypertension patients who complicted with atrial fibrillation were divided into atrial fibrillation group(n=464),others were divided into Non atrial fibrillation group(n=492).Collecting general information,biochemical indicators,ultrasonic testing results and electrocardiogram indicator of the research objects,using Excel software build database,using SPSS20.0 software for data analysis.If quantitative data conforms to normal distribution,it was described with mean ±standard deviation(x±S),else it was described with median.We using t-test for comparing two groups and single factor analysis of variance compared measurement data between three groups,and using LSD-t test compare make multiple comparison between several groups.Data does not obey normal distribution between groups was compared with two samples nonparametric tests.The comparison of the rate and the composition was performed by chi-square test.Multivariate analysis using Logistic regression analysis,statistics and other factors,explore the independent factors.If P<0.05,there was a statistical difference between the groups.Result:1.Comparison of clinical background informationIn the two groups,there were differences in the age of the subjects and the duration of hypertension,and the age of the patients with hypertension were higher than those in the non atrial fibrillation group.The two groups have many differences in the age stages,family history of hypertension,smoking and high blood pressure level of risk,the risk of atrial fibrillation group in the older stages of the population is larger,and it has more family history of hypertension and smoking person.2.Comparison of clinical correlation factors between AF group and non atrial fibrillation group(1)blood biochemical test results of two groups of BNP,CYS,TC,TG,HDL,LDL,Cr.LDL,BNP,CYS,Cr atrial fibrillation group was higher than non AF group,and the rest TC,TG,HDL atrial fibrillation group was lower than non atrial fibrillation group.(2)MA/U-Cre in urine were higher than that in non atrial fibrillation group.(3)the results showed that the PR interval duration was less than that of the non atrial fibrillation group.(4)the LAD,RVD,LVEF and LVPWd in cardiac ultrasound were different,and LVEF group was lower than that in non atrial fibrillation group,and LAD,RVD and LVPWd were higher than those in non atrial fibrillation group.(5)blood pressure monitoring results 5 indicators 24 hours average SBP,24 hours average PP,day PP,night SBP,night PP there are differences,atrial fibrillation group were higher than non atrial fibrillation group.(6)vascular ultrasound of the left carotid artery diameter,right common carotid artery diameter and right internal carotid artery intima-media thickness of the real fibrillation group higher than non AF group.3.atrial fibrillation related risk factors analysisIn order to control the function of other factors,Logistic regression analysis was used to explore the cause of the disease.Using the maximum and like a back step by step to select variables,introduced the level of 0.05,excluding the level of 0.10,single factor analysis meaningful factors,into the regression equation,eventually entering the equation of the factors have: left ventricular ejection fraction(LVEF),high density lipoprotein(HDL)is independent protective factor for hypertension complicated by atrial fibrillation and BNP,HR,lad,course of hypertension,age and PP,LDL is independent risk factors for hypertension complicated by atrial fibrillation.Conclusion:1.there is a family history of hypertension,smoking,age,the greater the duration of hypertension will increase the risk of hypertension associated with atrial fibrillation.2.hypertension and atrial fibrillation occurred risk and LDL,BNP,Cys,Cr,urine albumin and creatinine ratio,lad,RVD,lvpwd,24-hour average systolic blood pressure(SBP),24 hour average PP,PP,night SBP,PP night,left common carotid artery diameter,right common carotid artery diameter and right internal carotid artery intima-media thickness increased,and TC,TG,HDL,PR interval,LVEF decreased.3.controlling for confounding,LVEF,HDL is independent protective factor for hypertension complicated by atrial fibrillation;and LDL,BNP,LAD,course of hypertension,age and integrated PP is an independent risk factor for hypertension and atrial fibrillation.
