Objectives: 1.A quantitative study is conducted to investigate the current status of nurse-patient communication skills and to analyze its influencing factors in the breast department of a tertiary hospital in Shenyang,Liaoning Province.2.A qualitative study is conducted to further explore the influencing factors of nurse-patient communication skills training and the demand for training among nurses in the breast department.3.Based on the quantitative and qualitative research results,the training program of nursing-patient communication ability of nurses in the breast department is constructed,aiming at providing scientific guidance and reference for improving the effect of training on nursing-patient communication ability of nurses in the breast department.Methods: This study used a mixed research approach combining quantitative and qualitative research.1.Convenience sampling method was used to select 120 breast department nurses who met the inclusion criteria from three tertiary hospitals in Shenyang City.A general information questionnaire and a nurse-patient communication skills assessment scale were used to investigate the current status of nurse-patient communication skills and its influencing factors.Excel 2016 was used for data entry,and SPSS 25.0 software was used for data analysis.A significance level of α=0.05 was set,and P<0.05(two-tailed)indicated statistical significance.Descriptive statistics such as mean and standard deviation were used for continuous data,while frequency and composition ratios were used for categorical and ordinal data.Univariate analysis was conducted by using two independent sample t-tests or one-way analysis of variance(ANOVA)for variables that showed statistical significance(P<0.05).Variables with statistical significance were then used as independent variables,and the total score of nurse-patient communication skills in the breast department was used as the dependent variable.Multiple stepwise linear regression analysis was employed for multivariate analysis.2.A purposive sampling method was employed,following the principle of maximizing sample variation,to select 14 breast department nurses and 12 breast cancer patients for semi-structured interviews.This aimed to gain further insights into the influencing factors of nurse-patient communication skills training in the breast department and the demand for such training.The text data obtained from the interviews were coded,summarized,and thematic analysis was conducted using Nvivo 12 software.3.Based on a review of literature on training programs for improving nurse-patient communication skills both domestically and internationally,combined with the results of current situation surveys and qualitative interviews,a preliminary draft of the breast care nurse-patient communication skills training program has been formulated.Using the expert conference method,the preliminary training program will be revised and improved to create the final version of the breast care nurse-patient communication skills training program.Results: 1.Results on the current status of nurse-patient communication skills of nurses in breast department.(1)The total nurse-patient communication ability mean score among the nurses in the breast department was(4.19±0.36),indicatingaslightlyaboveaverage level.(2)The results of the univariate analysis showed that there were statistically significant differences in the communication skills of breast care nurses in terms of age,professional title,marital status,years of experience,position,teaching responsibilities,frequency of hospital communication training in the past 5 years,and frequency of communication training specifically related to breast care after joining the breast care department(P<0.05).(3)The results of multiple linear regression analysis showed that years of experience,position,frequency of hospital communication training in the past 5 years,and frequency of communication training specifically related to patient care after joining the breast care department were the variables included in the regression equation.This indicates that these four variables are influencing factors in the training of communication skills for breast care nurses,explaining a total of 47.2% of the variation in the model.2.Results of semi-structured interviews with breast care nurses and breast cancer patients.(1)Results of the qualitative study of nurses in the breast department:(1)Factors that facilitate nurse-patient communication;(2)Factors that hinder nurse-patient communication;(3)Demands for content in nurse-patient communication training;(4)Positive perception of nurse-patient communication training;(5)Recommendations for nurse-patient communication training.(2)Results of a qualitative study of breast cancer patients:(1)Information content desired by patients;(2)Need for caring communication;(3)Qualities that nurses should possess for effective communication.3.Results of constructing a training program for improving the communication skills of breast care nurses.The final draft of the training program included eight parts: training objectives,training content,communication templates for different scenarios,training methods,training duration,trainer’s qualifications,assessment methods,and case-based teaching.Conclusions: 1.The Breast care nurses’ communication skills with patients are slightly above average,but there is room for improvement.It is recommended to enhance the training of communication skills for breast care nurses.2.Age,position,frequency of hospital communication training in the past 5 years,and frequency of communication training specifically related to patient care after joining the breast care department are influencing factors for the communication skills of breast care nurses.These factors provide a basis for identifying the key target population for the construction of the training program aimed at improving the communication skills of breast care nurses.3.The qualitative research further uncovers the influencing factors on the communication skills of breast care nurses and their training needs,providing guidance for the construction of a training program focused on improving the communication skills of breast care nurses.The training program should emphasize the training of communication knowledge and skills,specialized knowledge on breast cancer,understanding the psychological responses of breast cancer patients,and delivering comprehensive information to patients.Additionally,it is advisable to have trainers with a background in psychology education,and consider online training methods for theoretical lectures.4.Based on the results of both quantitative and qualitative research,this study has developed a breast care nurse communication training program that is scientifically and practically feasible.The program includes eight components: training objectives,training content,communication templates for different scenarios,training methods,training duration,trainers’ qualifications,assessment methods,and case-based teaching.This program can serve as a guide for clinical training on the communication skills of breast care nurses and provide a foundation for conducting further research in this field.
