Objective: To observe the clinical efficacy of acupuncture combined with self-designed Xingong Headache Powder in patients with phlegm-turbid migraine.Observe the improvement of headache symptoms and TCM syndromes of patients,and compare the results with those of simple acupuncture or simple oral administration of self-designed Xingong Headache Powder,in order to enrich clinical treatment options.Methods: 87 patients who met the inclusion criteria were randomly divided into an acupuncture group,a herbal medicine group,and a combination acupuncture and herbal medicine group,29 cases in each one.Acupuncture group was treated with acupuncture,acupoint selection: the affected side takes Taiyang,Sizhukong penetrating Shuaigu,Waiguan,Xiaxi,bilateral Fengchi,Fenglong,Zhongwan and Ashi points.The above acupoints were treated with reinforcing-reducing manipulation until the sensation of qi was obtained.All acupoints were kept for 30 min,no needles are allowed during the period,and acupuncture was given once a day for 6 days and then one day was taken off.The herbal medicine group was given self-made Xin Gong Headache Powder,ground and took it with water,6g each in the morning and evening on an empty stomach for 7 days.Acupuncture combined with herbal medicine group was used in acupuncture combined with herbal medicine,and the acupuncture and medication methods were the same as those in acupuncture group and herbal medicine group.All the patients in the three groups received four courses of treatment.The same physician,who did not participate in the treatment,collected and evaluated VAS scores,headache symptom scores,TCM syndrome scores,and MIDAS scores for the three groups of patients before and after treatment,and followed up twice at 1 and 3months after treatment.Observe the clinical efficacy and long-term effects of combined acupuncture and self-designed Xingong Headache Powder in treating phlegm-stagnation type migraine.Results:1.VAS score analysisAfter the total course of treatment,the VAS scores of the patients in the three groups were all decreased significantly(P<0.05),while the VAS score of the acupuncture and herbal medicine combination group was decreased more significantly(P<0.05).These results indicated that all the three treatment methods could effectively relieve headache,but the efficacy of the acupuncture and herbal medicine combination group was superior to that of the other two groups.2.Headache symptom score analysisThe clinical efficacy of each treatment was evaluated by observing the attack times,duration,severity of headache,accompanying symptoms and headache symptom scores of the patients.After the end of the total course of treatment,the patients in the three groups showed significant decreases in the attack times,duration,severity of headache and accompanying symptom(P <0.05).Furthermore,the pairwise comparison among the three groups revealed that the reduction degrees of various indicators in the acupuncture-herbal medicine combination group were lower than those in the other two groups(P<0.05).In the within-group comparison of the twice follow-up visits,the differences between the three groups were not statistically significant(P>0.05),indicating that the three therapies had certain long-term efficacy.In the between-group comparison of the twice follow-up visits,the combination of acupuncture and herbal medicine group had a lower degree of reduction in headache symptom scores than the other two groups(P<0.05),suggesting that the acupuncture and herbal medicine combination group had the best improvement in patients’ headache symptoms and maintenance of long-term efficacy.3.TCM syndrome scoring analysisAfter the end of the total course of treatment,the TCM syndrome scores of patients with phlegm-turbid migraine were observed.Compared with those before treatment,the TCM syndrome scores of each group were significantly reduced(P<0.05).A further pairwise comparison among the three groups showed that the TCM syndrome scores of the combination group were lower than those of the other two groups(P<0.05).The inter-group comparison of the twice follow-up visits showed that the difference between the two follow-up visits of the three groups was not statistically significant(P>0.05),indicating that the three therapies had certain long-term efficacy.In the inter-group comparison of the twice follow-up visits,the reduction degrees of TCM syndrome scores in the acupuncture and herbal medicine combination group were lower than those in the other two groups(P<0.05),indicating that the acupuncture and herbal medicine combination group had the best efficacy in improving the TCM syndromes of patients and maintaining the long-term efficacy.4.MIDAS scoring analysisFor the three different schemes for the treatment of migraine,after four courses of treatment and twice long-term follow-up visits,the MIDAS scores of the three groups were significantly improved compared with those before treatment(P<0.05),and the quality of life was also improved.Further comparison showed that the improvement in the acupuncture-herbal medicine combination group was the best,and it was lower than that in the other two groups(P<0.05).These results indicated that the acupuncture combined with herbal medicine group significantly improved the migraine burden of the patients.5.Curative effect analysis of headache symptomIn the efficacy evaluation with headache symptom score as the main efficacy index,the effective rate was 88.47% in the acupuncture and herbal medicine combination group,74.07% in the acupuncture group,and 74.07% in the herbal medicine group.Statistical analysis of the effective rates among the three groups revealed that the acupuncture combined with herbal medicine group had the highest effective rate(P<0.05),indicating that the acupuncture combined with herbal medicine had the best curative effect.6.Analysis on curative effect of TCM syndromeIn the efficacy evaluation with TCM syndrome score as a secondary efficacy index,the efficacy of the acupuncture and herbal medicine combination group was 92.31%,that of the acupuncture group was 81.48%,and that of the herbal medicine group was 85.19%.The efficacy rate analysis through statistical analysis revealed that the efficacy rate of the acupuncture and herbal medicine combination group was the highest(P<0.05),indicating that the acupuncture and herbal medicine combination therapy had the best efficacy.Conclusion:1.Acupuncture combined with self-designed Xingong headache powder can alleviate the headache symptoms of patients with migraine,reduce the frequency and duration of migraine attacks,relieve the degree of headache,and alleviate associated symptoms.Its effect is better than that of acupuncture or oral administration of self-designed Xingong headache powder.2.Acupuncture combined with self-designed Xingong headache powder can improve the TCM syndrome and quality of life of patients with phlegm-turbid migraine.Its effect is better than that of acupuncture or oral administration of self-designed Xingong headache powder.3.Acupuncture combined with self-designed Xingong headache powder has long-term efficacy in treating patients with phlegm-turbid migraine,In terms of headache symptoms and phlegm syndrome,the long-term effect of acupuncture combined with herbal group was better than that of acupuncture or oral administration of self-designed Xingong headache powder. |
