Clinical Characteristics And Influencing Factors Of Ketosis Prone Type 2 Diabetes Patients

Objective: To retrospectively analyze the clinical data of newly diagnosed ketosis prone type 2 diabetes(KPD)and newly diagnosed simple type 2 diabetes(T2DM),compare the differences between the two and analyze the influencing factors.Methods: A total of 318 patients with T2 DM who were initially diagnosed in the Endocrinology Department of the Fifth Affiliated Hospital of Xinjiang Medical University from January2017 to June 2022 were collected as research subjects.According to whether or not the patients were diagnosed with ketosis,they were divided into two groups: 192 patients with T2 DM who were diagnosed with ketosis and 126 patients with pure T2 DM.The clinical data and influencing factors were analyzed.Results:The male proportion,length of stay,fasting blood glucose,2-hour blood glucose,glycosylated hemoglobin,creatinine,uric acid and fatty liver prevalence rate in ketosis prone type 2 diabetes group were higher than those in simple type 2 diabetes group.Age,course of disease,fasting C-peptide,2-hour C-peptide,fasting insulin,2-hour insulin,high-density lipoprotein,microalbuminuria,insulin resistance index,insulin sensitivity index,insulin secretion index The prevalence of carotid atherosclerosis and hypertension were lower than those of type 2 diabetes alone(P<0.05).After binary logistic regression analysis,there were statistically significant differences in the course of disease,glycosylated hemoglobin,and blood uric acid(P<0.05).The two groups were divided into subgroups according to glycosylated hemoglobin.In the ketosis prone type 2 diabetes group,HOMA-IR and HOMA-β between Hb Alc ≤ 11% and Hb Alc>11% subgroups.The difference was not statistically significant(P>0.05).In the simple type 2 diabetes group,HOMA-IR and HOMA-β of Hb Alc ≤ 11%subgroup is lower than that of Hb Alc>11 subgroup and Hb Alc ≤ 11% subgroup.It was significantly higher than Hb Alc>11% subgroup(P<0.05).The two groups were divided into subgroups according to age.In the ketosis prone type 2 diabetes group,HOMA-IR and HOMA-β between the subgroups Hb Alc ≤ 11% and Hb Alc>11%.The difference was not statistically significant(P>0.05).In the simple type 2 diabetes group,HOMA-IR and HOMA-β of Hb Alc ≤ 11% subgroup is lower than that of Hb Alc>11% subgroup.Hb Alc≤11% subgroup was significantly higher than Hb Alc>11% subgroup(P<0.05).Conclusion:There are statistical differences between the ketosis prone type 2 diabetes group and the simple type 2 diabetes group in the course of disease,glycosylated hemoglobin,and blood uric acid clinical data,suggesting that the ketosis prone type 2 diabetes patients have shorter course of disease,higher blood sugar at onset,and poor islet function.
