| Objective:To observe the clinical efficacy of modified Huanglian Ejiao decoction in the treatment of chronic kidney disease in stage 3-4 complicated with anxiety and TCM syndrome differentiation belonging to heart-kidney disharmony type.To explore the possible mechanism of modified Huanglian Ejiao decoction in the treatment of anxiety state.The purpose of this study is to provide ideas and experience of traditional Chinese medicine in the treatment of CKD complicated with anxiety in the future.Methods:From January 2022 to January 2023,70 outpatients and inpatients in the Department of Nephropathy of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine met the diagnostic criteria of chronic kidney disease stage 3-4 complicated with anxiety and TCM syndrome differentiation belonging to heart-kidney disharmony type.The patients were randomly divided into treatment group(n=35)and control group(n=35).Both groups were given basic treatment and psychological intervention.On this basis,the treatment group was given modified Huanglian Ejiao decoction in the morning and half an hour after dinner,150 ml each time,while the control group was given Wuling capsule,3 tablets each time,with a treatment period of 4 weeks.The changes of Hamilton anxiety scale score,renal function(blood Scr,blood BUN,Cysc,e GFR)and TCM syndrome scores before and after treatment were observed and statistically analyzed by SPSS26.0 software.Result:1.HAMA score:After treatment,the total scores of HAMA scale in the two groups were lower than those before treatment,and the total scores of HAMA,the score of physical anxiety factor and the score of mental anxiety factor in the treatment group were lower than those in the control group(P <0.05).2.TCM syndrome score:After treatment,the total score of TCM syndrome in the two groups was lower than that before treatment(P < 0.05),and the total score in the treatment group was lower than that in the control group(P < 0.05).The improvement effect of the treatment group on upset,insomnia,palpitation,forgetfulness,softness of waist and knees,hot flashes and night sweats was more obvious than that of the control group(P < 0.05).Compared with the two groups before and after treatment,there was no significant improvement in the symptoms of wet dreams and dizziness and tinnitus(P > 0.05).3.Renal function index:After treatment,the blood Scr,blood BUN and Cysc of the two groups decreased,and the level of e GFR increased.Compared with the control group,the improvement of renal function index in the treatment group was more obvious.4.Comparison of overall curative effect:In terms of anxiety state,the effective rate of the treatment group was 91.18%,and that of the control group was 81.82%.The therapeutic effect of the treatment group was better than that of the control group(P < 0.05).In the evaluation of the curative effect of traditional Chinese medicine,the effective rate of the treatment group was 85.29%,and that of the control group was 75.76%,the difference was statistically significant(P < 0.05).The improvement of clinical symptoms of patients with CKD3-4 complicated with anxiety and TCM syndrome differentiation belonging to heart-kidney disharmony type was better than that of the control group.Conclusion:1.Modified Huanglian Ejiao decoction can improve the clinical symptoms of patients with CKD3-4 complicated with anxiety and TCM syndrome differentiation belonging to heart-kidney disharmony type.2.Modified Huanglian Ejiao decoction can reduce the anxiety score of CKD3-4 patients and improve the renal function of CKD3-4 patients.3.Modified Huanglian Ejiao decoction can improve the quality of life of CKD3-4 patients and improve the comprehensive clinical effect. |
