Research On Professor Luo Weisheng’s Medication Rules And Academic Thoughts In The Treatment Of Non-alcoholic Fatty Liver Disease Based On Data Mining

Objective: By collecting and collating outpatient medical record data of Professor Luo Weisheng’s treatment of non-alcoholic fatty liver disease(NAFLD)and conducting retrospective study,this study analyzed and obtained Professor Luo’s drug use law in the treatment of NAFLD.To summarize Professor Luo’s academic thoughts on NAFLD treatment.Method: 106 patients with NAFLD who met the criteria of the study were collected from Professor Luo Weisheng’s outpatient department in Xianxi Hospital,the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine and Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine from January 2019 to October 2022.The basic information,symptoms and signs,and prescription drugs of the patients were input into Excel tables to establish a database.With the help of traditional Chinese medicine inheritance platform(V3.0),the "statistical analysis" function was used to count the frequency of patients’ gender,age,syndrome type,four qi and five taste of traditional Chinese medicine,channel conversion and classification,and the "symptom analysis","tongue image analysis" and "pulse analysis" function was used to count the frequency of patients’ symptoms,tongue image and pulse image respectively.The function of "prescription analysis" was used to conduct the statistics of the frequency of traditional Chinese medicine,drug association rules and drug cluster analysis,and to explore the medication rules of Professor Luo in the treatment of NAFLD.Results:1.Basic information: Among the 106 patients included,73 were male,accounting for 68.87%,and 33 were female,accounting for 31.13%;The youngest was 18 years old,the largest was 81 years old,mainly distributed in30-59 years old,accounting for 76.42%;The peak age is between 30 and 39 years for men and between 50 and 59 years for women.2.Symptoms and signs: there were 73 kinds of symptoms,with a total frequency of 543 times.The symptoms with high frequency were loose stool,dry mouth,bitter mouth,poor sleep,fatigue,anxiety and irritability,hypochondriac pain,abdominal distension and tightness,etc.There were 19 kinds of tongue images with a total frequency of 242 times.The tongue images with higher frequency were light red tongue,greasy white coating,thin white coating,light dark tongue and tooth marks on the edge.There are13 kinds of pulse,and the ones with high frequency are the sinking and fine pulse,the string,the small pulse,the smooth pulse and the smooth pulse.3.Syndrome type: There were 12 types of syndrome,mainly the syndrome of liver depression and spleen deficiency(34.91%),the syndrome of dampness and turbidity obstruction(30.19%),followed by the syndrome of liver qi stagnation(8.49%),the syndrome of spleen and stomach qi deficiency(6.60%)and the syndrome of liver and stomach heat stagnation(6.60%).4.Frequency and classification of Chinese medicine: A total of 162 traditional Chinese medicines were used in the 106 prescriptions included,excluding the high-frequency compound glycyrrhiza licorice,and the total frequency was2183 times.The top 20 medicines were Poria coia,white peony,white Rhizome,Tangerine peel,Pinellia,Magnolia officinalis,Salvia miltiorrhiza,Schisandra chinensis,Ligusticum chuanxiong,Astragalus,Bupleurum,Alismatis,Moutan bark,wood herb,Angelica sinensis,gardenia,black lotus,Lychee kernel,Money grass,cooked Rehmannia.There were 18 categories of TCM,mainly supplementing deficiency(20.88%),followed by promoting blood circulation and removing blood stasis(14.20%),improving water and dampness(13.73%),regulating qi(13.26%),clearing heat(7.81%),dispelling dampness(6.58%)and withdrawing astringent(5.40%).5.Four properties,Five flavors,meridian: the four properties are mainly warm(44.32%),cold(34.14%),flat(19.51%);the five flavors were mainly bitter(34.79%),sweet(28.60%)and sweet(23.58%);the meridian were mainly spleen channel(19.82%),liver channel(17.34%),followed by lung channel(13.81%),kidney channel(11.38%),heart channel(11.26%),stomach channel(10.94%).6.Drug association rule: 42 core drug combinations were obtained by setting the number of support as 43 and confidence as 0.9,including 24 two-flavor drug combinations,15 three-flavor drug combinations,and 3 four-flavor drug combinations.Thirty-two association rules of drug combination were obtained,including 7 association rules of second drug combination,17 association rules of third drug combination and 8 association rules of fourth drug combination.7.Drug cluster analysis: four core drug cluster combinations were obtained:Poria Cocos-Paeoniae paeoniae-Ligusticum Chuanxiong-Atractyloides-Schisandra-Astragalus,Magnolia officinalis-Tangerine peel-Pinellia pinellia-Poria cocos-Black medicine-areca nut,Poria Cocos-Pinellia pinellia-Tangerine peel-White Rhizome-Salvia miltiorrhiza-White Peony Root,Poria Cocos-White Peony-white Rhizome-Tangerine peel-Pinellia pinellia-Bupleurum.Conclusions: Professor Luo Weisheng advocates the idea of "treating non-alcoholic fatty liver disease from the spleen".He believes that improper diet damages the spleen and stomach,loss of qi and blood through work and dissipation,emotional disorders damage the liver and spleen,and congenital deficiency of phlegm-dampness constitution are common causes.In the early stage of the disease,qi deficiency of the spleen and stomach is the basis and qi stagnation of the liver is the standard.In the later stage,the disease involves the kidney,spleen and kidney deficiency,dampness and turbidness,phlegm and blood stasis accumulate in the liver.The principle of syndrome differentiation is "removing fat will strengthen the spleen,removing fat will guide stagnation,removing fat will remove turbidity,removing fat will remove blood stasis";The drugs were mainly used to soothe the liver and strengthen the spleen,dry dampness and reduce phlegm,regulate dampness and gasify dampness,supplement qi and blood,combine warm and cold,open bitterness and reduce bitterness,and pay attention to regulating the liver,spleen,stomach and kidney.Chaishao Liujunzi Decoction,Erchen decoction,Simo decoction,Pinellia Houpu Decoction and Bazhen decoction were used as the base formula,and the changes were made according to the syndrome.
