Objective: By comparing with Dieda ointment,which is most commonly used in our hospital,this paper aims to study the short-term clinical efficacy and safety of ironing therapy in the treatment of limb swelling and pain after total hip arthroplasty,in order to find a better treatment method.to provide reference for the clinical treatment of limb swelling and pain after total hip arthroplasty.Methods: This subject is a clinical prospective control trial.50 inpatients who underwent total hip arthroplasty in Ruikang Hospital of Guangxi University of traditional Chinese Medicine from January 2022 to December 2022 were selected.According to the different external application of traditional Chinese medicine after operation,the patients were randomly divided into two groups.25 patients in the observation group were treated with ironing therapy and 25 patients in the control group were treated with Dieda ointment.The NRS digital score and the changes of the circumference of the affected limb were recorded and compared between the two groups before operation and on the1 st,3rd,7th and 10 th day of external application.Record and compare the TCM syndrome scores of the two groups before operation,5 and 10 days after external application;record and compare the changes of hypersensitive CRP between the two groups before operation,5 days and 10 days after external application,and record and deal with adverse reactions.The data were sorted out and analyzed by statistical software SPSS25.0.Results: 1.Comparison of baseline data:(1)there were 3 and 2 patients in the ironing group and 2 patients in the control group(Dieda ointment),respectively,and 45 patients were finally completed.there was no significant difference in completion between the two groups(P > 0.05).(2)there was no significant difference in sex,location,distribution of disease,age,course of disease and BMI index between the two groups.2.Comparison of curative effect index:(1)there was no significant difference in preoperative NRS digital score,preoperative thigh root circumference,preoperative TCM syndrome score and preoperative hypersensitive CRP between the two groups(P > 0.05).(2)NRS score: in the intra-group comparison,there was no significant difference in NRS score between the two groups on the 1st day after external application and before operation,but on the 3rd,7th and 10 th day after external application,the NRS score in the two groups decreased significantly(P < 0.05).There were significant differences in NRS scores between the two groups on the 3rd day and the 3rd day,the 7th day and the 3rd day,and the 10 th day and the 7th day.In the comparison of simultaneous points between the two groups,there was no significant difference between the ironing group and the control group on the 1st and 3rd day of external application(P > 0.05),but on the 7th and 10 th day of external application,the ironing group was significantly better than that of the control group(P < 0.05).(3)Swelling degree: compared within the group,there were significant differences in the perimeter of the thigh root between the two groups on the 1st,3rd and 7th day after external application and before operation(P < 0.05).There were significant differences in the perimeter of the thigh root between the third day and the first day,the 7th day and the 3rd day,and the 10 th day and the 7th day after external application between the two groups(P < 0.05).On the 7th day of external application,the circumference of the thigh root of the ironing group was significantly lower than that of the control group(P < 0.05),and the therapeutic effect of the ironing group was better than that of the control group on the 7th day of external application.According to the change of perimeter difference to evaluate the degree of swelling of the affected limb before and after operation,the swelling of the two groups belonged to the third degree swelling on the first day,the second degree swelling on the third day and the first degree swelling on the seventh day.(4)TCM syndrome score: there was no significant difference between the two groups on the 5th day after external application(P > 0.05),and between the 10 th day after operation and the 5th day after external application(P < 0.05).There was significant difference between the two groups(P < 0.05).In the comparison of the two groups at the same time,there was no significant difference between the two groups on the5 th day of external application(P > 0.05).On the 10 th day of external application,the score of TCM syndrome in the ironing group was significantly lower than that in the control group(Dieda ointment)(P < 0.05).(5)hypersensitive CRP: there was significant difference in hypersensitive C-reactive protein between the two groups within 5 days and 10 days after external application,but there was no significant difference between the two groups within 5 days after external application.The hypersensitive C-reactive protein in the ironing group was lower than that in the control group within 10 days after external application,and the difference was statistically significant(P < 0.05).(6)there was significant difference in the curative effect of TCM syndrome between the two groups 10 days after operation(P < 0.05).The total clinical effective rate in the ironing group was significantly higher than that in the control group(82% < 74%).(7)there was no significant difference in allergic reaction between the two groups(P > 0.05).Conclusion(s): The main results are as follows:(1)both ironing therapy and Dieda ointment have a certain curative effect on the short-term recovery of pain and swelling of affected limbs after THA,and the clinical effect of ironing therapy is significantly better than that of Dieda ointment,and its total clinical effective rate is significantly higher than that of Dieda ointment.(2)the ironing therapy can significantly improve the symptoms and signs of the affected limb after THA,better reduce the early pain and swelling of the affected limb,better reduce the symptoms of ecchymosis and redness of the skin around the affected limb,and better reduce the serum inflammatory indexes.(3)ironing therapy has no serious adverse reactions in the course of treatment,and it has high safety,easy to use and good curative effect,so it is worthy of clinical application.
