Objective:To i nvestigate the effect of Bi ngnitrate Powder on lower limb swelling and pain after total knee arthroplasty,and to observe the clinical effect of Bingnitrate Powder on limb swelling after total knee arthroplasty.Methods:Sixty patients were selected from April 2017 to December 2018 in the First Affiliated Hospital of Shandong University of Traditional Chinese Medicine.According to the diagnostic criteria,inclusion criteria,exclusion criteria,exclusion criteria and abscission criteria,60 patients were randomly divided into self-made ice bag control group and glacial nitrate powder experimental group,30 in each group.The same nutrition and antithrombotic therapy were used in each group after operation.Bingniao powder was applied on both si des of the incision and inside and outside of the leg in the experimental group.Bingniao powder was separated by gauze and adhered to the skin.Each treatment lasted for 4 hours,twice a day.The patients in the control group were treated with self-made ice bag and the same position as the experimental group.The same treatment lasted for 4 hours,twice a day.The two groups were treated for 6 days.HSS functional score of knee joint was performed before operation and on the 3rd and 7th day after operation.The degree of swelling of affected limbs was measured on the 1st,3rd,5th and 7th day after operation.The circumference of 10 cm above patella,10 cm below patella and the midpoint of patella were measured.The VAS pain score of resting patients was also evaluated.The data were analyzed by SPSS19.0.Result:(1)There was no significant difference in swelling data between the two groups on the first day after operation(P>0.05),but there was significant difference in swelling data on the third,fifth and seventh day after operation(P<0.05).The degree of swelling in the experimental group was lower than that in the control group.(2)There was no significant difference in VAS score between the two groups on the first day after operation(P>0.05),but there was significant difference in VAS score on the third,fifth and seventh day after operation(P<0.05).The pain VAS score of the experimental group was lower than that of the control group.(3)There was no significant difference in HSS score of knee function between the two groups before operation(P>0.05).However,compared with preoperative,HSS score of knee joint improved significantly on the 3rd and 7th day after operation,with statistical difference(P<0.05).The HSS score of the experimental group and the control group on the 3rd and 7th day after operation also had statistical difference(P<0.05).The HSS score of the experimental group was higher than that of the self—made ice bag control group on the 3rd and 7th day after operation.Conclusion:(1)Bingnisan experimental group and sel—made ice bag control group have good therapeutic effect on swelling and pain after total knee arthroplasty,and they are effective treatment schemes.(2)The therapeutic effect of Bingnitrate Powder on swelling and pain after total knee arthroplasty is different between Bingnitrate Powder experimental group and self-made ice bag control group.By comparing the data,it is found that Bingnitrate Powder experimental group is better than self-made ice bag control group in all periods,which shows that Bingnitrate Powder external application has better therapeutic effect on swelling and pain of limbs after total knee arthroplasty.
