Objective: Based on the "Qi-blood theory",this study explores the application of Guipi Decoction in PO-circulation Vertigo(PCIV)with Qi-blood deficiency,to observe its clinical efficacy,clarify its safety,and explore the reasons for its effect.It provides reference for the treatment of PCIV with the combination of traditional Chinese and western medicine.Methods: A total of 90 patients with Qi-blood deficiency type PCIV who were admitted to the Department of Neurology,Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine from January 2020 to December 2020 and met the inclusion criteria were selected.According to the sequence of hospitalization in our hospital was coded,and randomly divided into treatment group and control group according to random number table method,with 45 cases in each group.Due to various reasons during the treatment process,2 patients in the treatment group and 2 patients in the control group fell off.Finally,43 patients in the treatment group and 43 patients in the control group completed the clinical study.All the enrolled patients were given conventional treatment of Qi and blood deficiency type PCIV,the control group was given vinpocetine injection on the basis of conventional treatment,and the treatment group was given Guipi Decoction on the basis of the control group for continuous treatment for 2 weeks.The vertigo disorder scale score,daily living ability score,TCM syndrome single score,TCM syndrome total score,transcranial Doppler ultrasound results of 2 groups before and after treatment were observed and recorded,and the clinical efficacy and safety of the two groups were evaluated.Vital signs,liver function,renal function,cardiac function,coagulation function,electrocardiogram,etc.,were monitored,and adverse reactions were observed.SPSS26.0 software was used for statistical analysis and processing of data.Results: 1.Before treatment,there was no statistical significance in gender,age,disease condition,risk factors,vertigo disorder scale score,ADL score,single score of TCM syndrome,total score of TCM syndrome and TCD result between the two groups(P>0.05),indicating comparability.2.Comparison of clinical efficacy: the effective rate of the treatment group was 90.70%,and that of the control group was 76.74%,indicating that the clinical efficacy of the treatment group was significantly better than that of the control group(P<0.05),and the difference was statistically significant.3.Comparison of vertigo disorder scale scores: After treatment,the vertigo disorder scale scores of the treatment group were significantly lower than those of the control group(P < 0.05),and the difference was statistically significant.4.Comparison of ADL scores: After treatment,the ADL score of the treatment group was significantly lower than that of the control group(P< 0.05),and the difference was statistically significant.5.Comparison of single item scores of TCM symptoms:(1)The scores of heart palpitation and less sleep,fatigue,complexion white,abdominal distention and less,tiredness and languor,lip nail and no China of Qi and blood deficiency before and after treatment in the treatment group were tested by matched sample t test(P < 0.05),and the differences were statistically significant,indicating that the treatment group could effectively improve the symptoms.(2)Paired sample t test(P>0.05)was performed for the scores of fatigue,abdominal distention,and labial nail irradiance in the control group before and after treatment,and the differences were not statistically significant,indicating that fatigue,abdominal distention,and labial nail irradiance could not be improved in the control group.(3)After treatment,the two groups of patients in palpitation,less sleep,fatigue,complexion white,abdominal distension and less,languor,labia not Hua Qi and blood deficiency symptom scores were independent sample T test(P< 0.05),the difference was statistically significant.6.Comparison of total scores of TCM symptoms: After treatment,the total scores of TCM symptoms in both groups were lower than those before treatment(P< 0.05),and the difference was statistically significant.The reduction of total scores of TCM symptoms in the treatment group was significantly better than that in the control group(P < 0.05),and the difference was statistically significant.7.Comparison of TCD results: Chi-square test was performed on the TCD results before and after treatment in 2 groups(P< 0.05),and the difference was statistically significant.That is,the number of blood vessels with reduced flow rate was significantly reduced in the treatment group and the control group after treatment.Conclusions: 1.The application of Guipi Decoction combined with western medicine in the treatment of Qi and blood deficiency type PCIV showed that its clinical effect was better than that of western medicine alone,and it could improve the daily living ability of patients.2.The application of Guipi Decoction combined with western medicine can improve the symptoms of Qi and blood deficiency such as palpitation and less sleep,fatigue,abdominal distension and less sleep,complexion white,tiredness and lazy speech,and lip Jia Hua,indicating that Guipi Decoction can improve the clinical symptoms of patients and improve their quality of life from an overall point of view.3.TCD results showed that Guipi Decoction combined with western medicine can improve the blood flow number of PCIV arteries and the blood supply to the brain of the type of Qi and blood deficiency,which may be the reason for the prevention and treatment of the attack of ischemic vertigo in the posterior circulation.4.In this clinical study of Qi and blood deficiency type PCIV,no toxic and side effects of Guipi Decoction were observed.It is safe and effective and worthy of clinical promotion.5.With the deepening of related studies,Guipi Decoction combined with Western medicine can be used as a treatment for PCIV with deficiency of qi and blood.The complementary advantages of Chinese and Western medicine should be paid attention to and the path of integrated Chinese and Western medicine should be followed. |
