Objective: Based on the theory of etiology and pathogenesis in Chinese medicine,the post-arthroscopic swelling and pain after knee meniscus injury was regarded as "tendon injury".Compared with the conventional postoperative treatment and rehabilitation,we investigated whether the addition of modified Xuefuzhuyu decoction can more effectively relieve the postoperative symptoms and promote the functional recovery of the knee joint after arthroscopy under the treatment concept of activating blood,promoting “qi” and dispelling dampness..Methods: In this study,70 patients who underwent arthroscopic meniscus repair at Shandong Provincial Hospital of Traditional Chinese Medicine from November 2021 to December 2022 were selected for a comparative study using a randomized group.In the control group,35 patients underwent arthroscopic exploration and meniscus injury repair with routine postoperative treatment care,and a series of small-load rehabilitation exercises such as ankle pump training,quadriceps isometric contraction functional exercise,straight leg raising functional exercise,and active knee flexion and extension exercise will also be performed.In the observation group,the treatment plan of the control group was supplemented with the Chinese herbal remedy modified Xuefuzhuyu decoction in order to improve the symptoms of swelling,pain,and unfavorable activity after knee arthroscopy and improve the functional score of the affected knee.Prior to surgery,the investigators assessed the degree of swelling of the affected knee,maximum angle of active flexion and extension activity,the visualized pain scale(VAS)and the knee Lysholm scoring system by using tools and forms to assess the patient’s condition.Also note the enrollment of detached patients and the occurrence of adverse drug events.After all study subjects were followed up,the data of comparative observation indexes were calculated and analyzed using the statistical analysis software SPSS 24.0.Results: The baseline differences in the general profile of patients in the experimental and control groups recorded preoperatively were not statistically different(P>0.05)and were comparable.1.Swelling degree The difference was statistically significant on postoperative day 1(3.74±0.50)compared with day 7(2.09±0.28)in the experimental group(t=19.508,P<0.001),and on postoperative day 1(3.74±0.50)compared with day 14(1.00±0.29)in the experimental group(t=28.880,P<0.001),the difference The difference was statistically significant;in the control group,the difference was statistically significant on postoperative day 1(3.74±0.50)compared with day 7(2.09±0.28)(t=17.070,P<0.001),and in the experimental group,the difference was statistically significant on postoperative day 1(3.74±0.50)compared with day 14(1.00±0.29)(t=29.148,P< 0.001),the difference was statistically significant.While the experimental group was compared with the control group on postoperative day 7(t=-4.297,P<0.001)and postoperative day 14(t=-4.779,P<0.001),the difference between the two data groups was significant.This indicates that the postoperative joint swelling was significantly improved in both groups,and the degree of improvement was more significant in the experimental group.2.Mobility The difference was statistically significant when comparing the experimental group on postoperative day 1(24.74±6.91)with day 7(68.17±8.35)(t=-22.145,P<0.001),and the difference was statistically significant when comparing the experimental group on postoperative day 1(24.74±6.91)with day 14(92.14±11.13)(t=-29.896,P< 0.001),the difference was statistically significant;in the control group,the difference was statistically significant on postoperative day 1(24.83±6.55)compared with day 7(58.03±6.60)(t=-25.586,P<0.001),and in the experimental group,the difference was statistically significant on postoperative day 1(24.83±6.55)compared with day 14(83.23±8.82)(t =-32.714,P<0.001),the difference was statistically significant.While the experimental group was compared with the control group on postoperative day 7(t=5.641,P<0.001)and postoperative day 14(t=3.715,P<0.001),the difference between the two data groups was significant.This indicates that the postoperative joint mobility was significantly improved in both groups,and the improvement was more obvious in the experimental group.3.VAS score The difference was statistically significant when comparing [8(7,9)] on postoperative day 1 with [4(3,5)] on postoperative day 7(Z=5.186,P<0.001)and [8(7,9)]on postoperative day 1 with [2(1,3)] on postoperative day 14(Z=5.178,P<0.001)in the experimental group.In the control group,the difference was statistically significant on postoperative day 1 [8(7,9)] compared with day 7 [6(5,7)](Z=4.561,P<0.001),and in the experimental group,the difference was statistically significant on postoperative day 1[8(7,9)] compared with day 14 [3(2,4)](Z=5.195,P<0.001).The differences were significant in the experimental group compared with the control group on postoperative day 7(Z=170.000,P<0.001)and day 14(Z=259.000,P<0.001).In contrast,there was a significant difference between the experimental group and the control group comparing postoperative day 7(Z=170.000,P<0.001)and day 14(Z=259.000,P<0.001).This indicates that postoperative joint swelling was significantly improved in both groups,and the degree of improvement was more significant in the experimental group.4.Lysholm score of knee The difference was statistically significant when comparing the experimental group on postoperative day 1(18.37±2.94)with day 7(50.60±5.51)(t=-31.698,P<0.001)and the experimental group on postoperative day 1(18.37±2.94)with day 14(76.74±6.61)(t=-44.107,P<0.001),the difference was statistically significant;in the control group,the difference was statistically significant when comparing postoperative day 1(18.26±2.89)with day 7(46.51±6.84)(t=-22.733,P<0.001),and in the experimental group,the difference was statistically significant when comparing postoperative day 1(18.26±2.89)with day 14(72.86±8.67).8.67)in the experimental group(t=-33.536,P<0.001),the difference was statistically significant.The difference between the experimental group and the control group was statistically significant(P<0.05)on postoperative day 7(t=2.753,P=0.008)and on postoperative day 14(t=2.108,P=0.039).5.Analysis of postoperative knee outcome indexes The total effective rate of the experimental group(91.43%)was greater than the total effective rate of the control group(80.00%),in which the differences between the healed,effective,efficient and invalid values of the two groups of patients were statistically significant(Z=448.500,P=0.038 <0.05).Conclusion: After arthroscopic repair of knee meniscus injury,the use of modified Xuefuzhuyu decoction can reduce swelling and pain,increase mobility,improve joint function and promote recovery as early as possible.
