Objective: To study the efficacy of acupuncture and drug combination in the treatment of increased muscle tone in the convalescent period of cerebral infarction under the guidance of the theory of " Nutrient Qi and Defensive Qi ",and to provide a basis for its clinical promotion.Methods: According to the inclusion and exclusion criteria,99 patients with hypertonia in the recovery stage of cerebral infarction in the Department of Classical Chinese Medicine,Affiliated Hospital of Shandong University of Chinese Medicine from November 2021 to February 2023 were selected as the research objects.According to the random table method,they were divided into a combined acupuncture and medication group,a simple acupuncture group and a conventional treatment group,with 33 cases in each group.The usual treatment group was given injection blood plug intravenous drip and ethylperisone hydrochloride tablets orally;The combination of acupuncture and medicine treatment group was given internal administration of traditional Chinese medicine and acupuncture treatment on the basis of conventional treatment.In the acupuncture group alone,acupuncture was given in addition to usual treatment.Ingredients: Laurel15 g,White Peony 15 g,Astragalus 18 g,Angelica 12 g,Dried Ginger 6g,Licorice 6g;Acupuncture points:LI15,LI14,LI10,LI4,LI5,ST32,ST36,SP10,GB39,GB34,ST40.The patient’s muscle tone status was assessed by the modified Ashworth Spasticity Rating Scale(MAS);The simplified FuglMeyer Motor Function Scale(FMA)assesses the motor function of the upper limbs.Before treatment and 2 weeks after treatment,patients were evaluated for their efficacy in terms of muscle tone status and limb motor function,and the safety of this study was evaluated before and after treatment.Results:1.In this project,99 patients with increased muscle tone during the recovery period of cerebral infarction from November 2021 to February 2023 in the inpatient department of the Department of Traditional Chinese Medicine of our hospital were selected,and 91 cases were finally selected,including 52 males and 39 females,with a male-to-female ratio of 1.3:1,and the number of males was higher than female.There were no significant differences in gender,age and disease course between the three groups(P>0.05).2.The average age of the selected patients was 56.63±11.29.Among them,the youngest age was 38 years old,the oldest age was 75 years old,and there were 59 patients over 50 years old,accounting for 64.84% of the total,which was consistent with the age of onset of cerebral infarction.3.The total effective rates of the three groups were 88.67%,77.42%and 63.33%,respectively,and there were significant differences in the ANOVA of efficacy(P=0.00<0.01),and the acupuncture group was superior to the acupuncture group alone,and the acupuncture group was better than the conventional treatment group.It was explained that the addition and subtraction of guizhi soup combined with acupuncture treatment,acupuncture alone and conventional drug treatment could promote the recovery of limb motor function and reduce muscle tone.However,guided by the theory of "Nutrient Qi and Defensive Qi ",Guizhi soup plus minus combined with acupuncture treatment can better improve clinical symptoms and improve the quality of life of patients.4.Simplified FMA scores before and after treatment: The simplified FMA scores of the three groups before treatment were analyzed by ANOVA,P>0.05,and the three groups could be compared;after the combination of acupuncture and drug,acupuncture and conventional drug treatment,the simplified FMA scores of the three groups were higher than those before treatment,and the simple FMA scores of the three groups before and after treatment were tested by rank sum test(P<0.01),indicating that the above three treatments could effectively improve the motor function of the upper limbs.The rank sum test(P<0.05)and the comparison of the difference between the three groups before and after treatment(P<0.01)revealed that the combination of acupuncture was superior to acupuncture alone and acupuncture alone was superior to conventional treatment in improving upper limb motor function.Conclusion:1.The combination of injection and drug treatment is well tolerated in patients with increased muscle tone in the recovery period of cerebral infarction.Under the guidance of the " Nutrient Qi and Defensive Qi " theory,acupuncture combined treatment,simple acupuncture treatment,and conventional drug treatment can increase the simplified FMA score of the upper limbs in patients with increased muscle tone in the recovery period of cerebral infarction,reduce the level of muscle tone(MAS),and improve the motor function of the upper limbs.2.Under the guidance of the " Nutrient Qi and Defensive Qi " theory,the effect of guizhitang combined with acupuncture is better than that of acupuncture alone,and acupuncture treatment alone is better than conventional drug treatment.The combination of acupuncture and medicine guided by the theory of " Nutrient Qi and Defensive Qi " is an effective way to treat ischemic stroke with integrated traditional Chinese and Western medicine,and it is worth further promoting and applying.
