| Objective: To observe the clinical efficacy of nourishing Yin and Bushen formula in the treatment of decreased ovarian reserve(DOR)and its effect on the level of T lymphocyte subsets,to evaluate the clinical efficacy of nourishing Yin and Bushen formula, and to provide a new research method and theoretical basis for the treatment of DOR with traditional Chinesemedicine. Methods: In this study,62 patients with DOR were selected, all of which met the diagnostic criteria, inclusion criteria and exclusion criteria, and were randomly divided into 31 cases in two groups. The treatment group was treated with Yangyin Bushen decoction, and the control group was treated with Kuntai capsule, and treated with 3 menstrual cycles. The changes of follicle-stimulating hormone(FSH),follicle-stimulating hormone/luteinizing hormone(FSH/LH),anti-Mullerian hormone(AMH),antral follicle number(AFC),T lymphocyte subsets(T lymphocyte percentage,CD4~+percentage,CD4~+/CD8~+ratio),menstrual(main symptom)score,TCM symptom score and other indicators before and after treatment were observed to evaluate the clinical efficacy and advantages of the two groups in the treatment of DOR,which laid a foundation for the wide application of TCM in clinical practice. Results: During the study,1 case fell off in each group due to uncontrollable factors, and 60 cases were actually involved.60 patients were randomly divided into two groups with 30 patients in each group.(1)Sex hormones and AMH: After treatment,The two groups of patients could reduce FSH and FSH/LH ratio, and increase AMH(P<0.05),indicating that the efficacy of regulating hormone levels in the treatment group was more significant, and could significantly reduce the ratio of FSH and FSH/LH(P<0.05),and the effect of AMH treatment in the two groups had no significant effect(P>0.05);(2)Ovarian AFC: After treatment, both groups of patients could increase AFC(P<0.05),and the treatment effect in the treatment group was better than that in the control group(P<0.05);(3)T lymphocyte subsets: After treatment, the two groups of patients had the effect of increasing the percentage of T lymphocytes,CD4~+ and CD4~+/CD8~+ratio(P<0.05),indicating that the treatment group had a more significant effect on regulating the body’s immune function(P<0.05);(4)Menstrual(main symptom)score and TCM symptom score: After treatment, both groups of patients could increase menstrual(main symptom)score(P<0.05)and reduce TCM symptom score(P<0.05),indicating that the treatment group was better than the control group in adjusting menstruation and improving perimenopausal symptoms such as upset, insomnia, hot flash night sweat(P < 0.05);(5)Efficacy analysis: the total effective rate of menstrual(main symptom)points in the treatment group was 96.7%,the control group was 76.7%,and the total effective rate of menstrual(main symptoms)points in the treatment group was more significant(P<0.05);the total clinical effective rate of the treatment group was 96.6%,and the control group was 90%,and the clinical efficacy of the treatment group was better(P<0.05)compared with the two groups. Conclusion: Traditional Chinese medicine nourishing Yin and Bushen formula for the treatment of kidney Yin Deficiency DOR can effectively improve the clinical symptoms of menstrual disorders and a series of perimenopausal periods, and reduce serum FSH and FSH/LH ratio and increase AMH by regulating endocrine status and improving hormone levels.By regulating the level of T lymphocyte subsets, promoting T lymphocytes,CD4~+ and CD4~+/CD8~+ ratios to reach the balance of the body’s immune function, and regulating the body to prolong the cycle of granulosa cell apoptosis, inhibiting the progressive decline of ovarian function,so as to protect and improve ovarian reserve function. This scheme’s clinical efficacy is superior to Kuntai capsule, and the adverse reactions are smaller, and suitable for clinical application, which provides a rich theoretical basis and clinical practice experience for the treatment of DOR and the regulation of T lymphocyte subsets function by traditional Chinese medicine. |
