Objective: Under the guidance of the idea of combining traditional Chinese and western medicine and attaching equal importance to traditional Chinese and western medicine,this study intends to explore an effective treatment method for the local swelling of Pilon fracture during the peri-operation period,systematically study the effect of Taohong Siwu Decoction on the local limb swelling of Pilon fracture during the peri-operation period,and further evaluate the feasibility of Taohong Siwu Decoction in promoting the local limb swelling of Pilon fracture and reducing the occurrence and development of local skin related diseases during the peri-operation period,As well as the safety of clinical application of Taohong Siwu Decoction,in order to facilitate wider clinical application and further research.Methods: With the approval of the Ethics Committee of our hospital,a prospective,randomized and controlled study was adopted to select 62 Pilon fracture patients aged between 22 and 60 years old who were treated in the Department of Orthopedics and Traumatology,Affiliated Hospital of Traditional Chinese Medicine,Southwest Medical University,who underwent delayed open reduction and internal fixation.According to the order of admission of patients,the number is numbered according to the random number table method.The even number of patients are selected as the observation group,and the odd number of patients are selected as the control group.There are 31 patients with Pilon fracture in each group.The two groups of Pilon fracture patients were treated by the same group of tube bed doctors.The control group patients were treated with conventional western medicine.The observation group was treated with Taohong Siwu Decoction on the basis of the treatment of the control group.The swelling grade,swelling value,detumescence time,detumescence speed,occurrence rate of tension blister,occurrence rate of serious adverse events and general demographic data of the two groups were observed and analyzed.The clinical data of 62 patients with Pilon fracture were entered into Excel by single recording and double checking,and the relevant data were analyzed and processed by SPSS 20.00.The measurement data such as detumescence time are expressed by(mean ±standard deviation).If the measurement data such as detumescence time is skewed distribution and cannot meet the homogeneity of variance,the rank sum test is used for comparison between groups;If the measurement data such as detumescence time is normal distribution and meets the homogeneity of variance,the t-test of two independent samples shall be performed for comparison between groups.The occurrence rate of tension blister was expressed by the rate(%),and the comparison between groups was performed by the paired four-grid chi-square test.When P<0.05,there is a significant difference in clinical efficacy between the two groups of Pilon fracture patients,with statistical significance.Results:(1)The starting and ending dates of this study were from May 21,2022 to January 31,2023.A total of 62 cases of Pilon fracture were collected.During the study,2 cases fell off and 60 cases were actually completed.(2)The two groups of Pilon fracture patients had no statistical difference in the comparison of general data such as sex ratio of men and women,average age,fracture R ü edi-Allgower classification,soft tissue Tscherne classification,and injury cause,which was comparable(P>0.05).(3)The average swelling values of D0,D4 and Dz of Pilon fracture patients in the two groups were not statistically different(P>0.05),while the average swelling values in the D6 period were significantly better in the observation group than in the control group(P<0.05);In the comparison of swelling grading of Pilon fracture patients in the two groups,D0,D2,D4 and D8 had no statistical difference(P>0.05),while in the comparison of swelling grading in D6,the observation group was significantly better than the control group(P<0.05).In the intra-group comparison,it can be found that the average swelling values of D0 and Dz of Pilon fracture patients in the two groups have significant statistical differences(P<0.05).(4)The average detumescence time of Pilon fracture patients in the observation group was(5.16±1.05)days,which was significantly better than that of the control group(5.92±1.01)days,with a statistically significant difference(P<0.05).(5)The average detumescence rate of Pilon fracture patients in the observation group was(0.67±0.06)cm/d,which was significantly higher than that in the control group(0.54±0.07)cm/d,and the difference was statistically significant(P<0.05).(6)The incidence of tension blisters in Pilon fracture patients in the observation group was 23.33%,which was similar to 26.67% in the control group,with no significant difference(P>0.05).(7)During the hospitalization of Pilon fracture patients in both groups,there were no adverse events such as severe phlebitis,allergy to traditional Chinese and western medicine,liver and kidney damage,and severe water,salt and electrolyte disorder.Conclusion: Taohong Siwu Decoction combined with conventional western medicine for detumescence treatment,a combination of traditional Chinese and western medicine for detumescence treatment,is effective and safe in preventing the preoperative swelling of Pilon fracture.It can accelerate the detumescence speed of the swelling limb at the distal end of the lower leg and achieve the rapid rehabilitation purpose of shortening the detumescence time,which is worthy of promotion.
