Clinical Study Of Guben Changning Decoction On Treating Diarrhea-predominant Irritable Bowel Syndrome (Spleen And Kidney Yang Deficiency Syndrome)

Objective:To observe the clinical efficacy of Gubenchangning Decoction in treating diarrhea-predominant irritable bowel syndrome(IBS-D)with syndrome of spleen and kidney yang deficiency,through Contrasting the traditional Chinese medicine(TCM)syndromes score,IBS-SSS,IBS-QOL,HAMA before and after treatment.Methods:72 patients with IBS-D(syndrome of spleen and kidney yang deficiency)were randomly divided into the experimental group and the control group,36 cases in each group.The experimental group took Gubenchangning Decoction,and the control group took Shenbeiguchang Capsule.The treatment course was 4 weeks.After 0,2 and 4 weeks of treatment,the TCM symptom score,IBS-SSS,IBS-QOL and HAMA of the two groups of patientswere record,and data were statistically analyzed,At the same time,the safety indicators of the patients(liver function,renal function,blood routine,urine routine,stool routine and electrocardiogram)were recorded before and after treatment.Results:1.General data comparisonA total of 70 patients were included in the statistics,there were 36 cases in the experimental group and 34 in the control group.Before treatment,there was no significant difference in TCM syndromes,IBS-SSS,IBS-QOL,HAMA scores between the two groups(P>0.05),they were comparable.2.Comparison of the efficacy of TCM syndromesAfter the treatment,the total effective rate of the experimental group was 91.7%,and the total effective rate of the control group was 70.6%.The experimental group was better than the control group(P<0.05).3.Comparison of TCM syndromesAfter 2 weeks:the scores and total scores of the experimental group were all lower than those before treatment(P<0.05),and the scores and total scores in the control group were lower than those before treatment,except for loss of appetite(P<0.05).The scores and total scores were significantly lower than that of the control group(P<0.05).After 4 weeks:Compared with before treatment,the scores and total scores of the two groups were decreased(P<0.05).Compared with 2 weeks later,the scores and total scores of the experimental group were decreased(P<0.05).The scores of stool diarrhea,soreness and weakness of waist and knees syndrome and total scores in the control group were decreased(P<0.05).4.IBS-SSS comparisonAfter 4 weeks,the total scores of IBS-SSS in the two groups were significantly lower than those before and after 2 weeks of treatment,After 2 weeks and 4 weeks,the total score of IBS-SSS in the experimental group was lower than that in the control group(P<0.05).5.IBS-QOL comparisonAfter 2 weeks:the scores and total scores in the experimental group were higher than those before treatment,except for the scores of self-consciousness(P<0.05),the scores and total scores in the control group were higher than those before treatment,except for the scores of dysthymia and sexual behavior(P<0.05).Dysthymia,eating avoidance dimension scores and total scores in the experimental group were higher than those in the control group(P<0.05).After 4 weeks:compared with before treatment,the scores of all dimensions and total scores in both groups were improved(P<0.05);compared with 2 weeks later,the scores of dysthymia,health concern,social function,relationship expansion dimension and total score were improved in both groups(P<0.05);The scores and total scores of all dimensions in experimental group were higher than those in control group(P<0.05).6.HAMA comparisonAfter 4 weeks,the HAMA total score in the two groups were significantly lower than before treatment,after 2 weeks later,and significantly lower than before treatment after 2 weeks(P<0.05).7.SecurityThere were no abnormal changes in the safety indicators in the two groups before and after treatment.Conclusion:1.Guben Changning Decoction can improve the clinical symptoms of the IBS-D patients with spleen-kidney-yang deficiency syndrome.2.Guben Changning Decoction can reduce the severity of the IB S-D patients with spleen-kidney-yang deficiency syndrome.3.Guben Changning Decoction can improve the quality of life of the IBS-D patients with spleen-kidney-yang deficiency syndrome.4.Guben Changning Decoction can improve the mental and psychological state of the IB S-D patients with spleen-kidney-yang deficiency syndrome.
