| Objective:This study mainly discusses the short-term clinical efficacy and safety performance of Zhongfeng Tanyu Decoction on patients with phlegm Blocking Collaterals in acute ischemic stroke through theoretical research,clinical treatment effect observation and safety evaluation,so as to provide a theoretical basis for Zhongfeng Tanyu Decoction in the treatment of patients with phlegm Blocking Collaterals in acute ischemic stroke.And analyze its effect on the levels of platelet-derived growth factor BB(PDGF-BB)and angiopoietin 2(Ang-2)in angiogenesis related factors,and elaborate the action mechanism of Zhongfeng Tanyu Decoction in the treatment of acute ischemic stroke from the aspect of angiogenesis,so as to provide new ideas for the clinical treatment of acute ischemic stroke.Methods: 80 patients with acute ischemic stroke who met the inclusion criteria in the Department of Neurology of Liuzhou traditional Chinese medicine hospital(Liuzhou Zhuang Medical Hospital)from February 2021 to February 2022 were randomly divided into control group and treatment group,with 40 cases in each group.The control group was given routine basic treatment: general treatment in acute stage,anti platelet aggregation,lipid regulation,plaque stabilization and complications,treatment of chronic diseases,etc.The treatment group was given Zhongfeng Tanyu Decoction on the basis of routine basic treatment.The drug composition included Pinellia ternata 10 g,Atractylodes macrocephala 10 g,Gastrodia elata 15 g,Poria cocos15 g,tangerine peel 10 g,Fructus aurantii 10 g,Magnolia officinalis 10 g,Ligusticum chuanxiong 12 g,earthworm 12 g,safflower 6g,peach kernel 9g,Acorus calamus 15 g,rhubarb 10 g,licorice tablet 6g,ginger 6g and sandalwood 2g.One dose per day,100 ml each time,warm take twice in the morning and evening,and the course of treatment is 14 days.The short-term clinical efficacy and TCM symptom score of the two groups were observed and compared.The changes of stroke related scores(NIHSS,m RS,ADL)and the contents of PDGF-BB and Ang-2 in peripheral serum were compared between the two groups before and after treatment.The safety of AIS patients during treatment was evaluated.Results:There was no significant difference in gender,age,hypertension,diabetes,hyperlipidemia,hyperhomocysteinemia,hyperuricemia and smoking history between the two groups before treatment(P>0.05).(1)TCM syndrome score: the TCM syndrome scores of the two groups were improved before and after treatment,and there was difference between the two groups before and after treatment(P < 0.05).Compared between the two groups,the improvement of TCM syndrome score in the treatment group was more significant than that in the control group(P < 0.05).(2)Clinical efficacy of TCM syndrome: after treatment,the clinical effective rate of TCM syndrome in the treatment group was 90.00%,and that in the control group was 67.50%.The curative effect of the treatment group was better than that of the control group.(3)NIHSS score: the NIHSS score of the two groups decreased after treatment(P < 0.05),and the reduction range of the treatment group was better than that of the control group(P < 0.05).(4)MRS score:the MRS score between the two groups decreased after treatment,and the difference was statistically significant(P < 0.05).The treatment group was more significant than the control group,and the difference was statistically significant(P < 0.05).(5)ADL score: the independent living ability of the two groups increased,and the difference was statistically significant(P < 0.05).The treatment group was better than the control group(P < 0.05).(6)Angiogenesis related factors: after treatment,the levels of PDGF-BB and Ang-2 in the two groups were significantly higher than those before treatment(P < 0.05).The levels of PDGF-BB and Ang-2 in the treatment group were significantly higher than those in the control group(P < 0.05).(7)Safety: there was no significant difference in adverse reactions between the treatment group and the control group(P < 0.05).There were no serious adverse reactions related to the study in both groups.Conclusion:Clinical studies have found that Zhongfeng Tanyu decoction combined with conventional western medicine is effective in the treatment of acute ischemic stroke patients with phlegm blocking collaterals.It can improve the symptoms of traditional Chinese medicine,improve the neurological deficit,promote the recovery of independent living ability and improve the quality of life,and has good safety.Its mechanism may be to promote angiogenesis and improve brain blood supply by up regulating the expression of PDGF-BB and Ang-2 after cerebral ischemia injury. |
