Objective: To evaluate the clinical efficacy and safety of Chuanhong Zhongfeng Capsule in the treatment of ischemic stroke(blood stasis syndrome);to explore the neuroprotective effect of Chuanhong Zhongfeng Capsule on ischemic stroke rats;to further explore the mechanism of Chuanhong Zhongfeng Capsule in the treatment of ischemic stroke by combining network pharmacology and transcriptome technology.Methods: 1.The retrospective cohort study method was used to compare the mRS score,NIHSS score,the incidence of complications in patients with ischemic stroke(blood stasis syndrome)treated with Chuanhong Zhongfeng Capsule and patients with non Chuanhong Zhongfeng Capsule,the improvement of hemorheology,blood stasis syndrome and combined symptoms.2.The rats were randomly divided into blank group,model group,Chuanhong Zhongfeng Capsule group(treatment group)and Aspirin enteric coated tablet group(control group).After 28 days of continuous intragastric administration,the eyeball blood was taken,the plasma was centrifuged,and the brain was decapitated for HE staining,NO,SOD detection and transcriptomics determination;3.The target of Chuanhong Zhongfeng Capsule was predicted by network pharmacology,and the target related to ischemic stroke was screened out.Go analysis and KEGG enrichment analysis were carried out to build a visual network;4.Using transcriptome sequencing method to screen the differential genes of Chuanhong Zhongfeng Capsule in the treatment of ischemic stroke,and finally combined with the network pharmacology prediction results to select the key targets and pathways that may play a role in ischemic stroke.Results: 1.In the clinical study,compared with the control group,patients in the Chuanhong Zhongfeng Capsule treatment group had a therapeutic effect,and the combined symptoms improved well(P<0.05).2.In animal experiments,compared with the model group,Chuanhong Zhongfeng Capsules can significantly inhibit serum NO levels and increase SOD levels(P<0.05).3.Network pharmacology research shows that a total of 48 active ingredients have been identified in Chuanhong Zhongfeng Capsules,mainly including flavonoids,organic acids,glycosides,and other compounds,and 74 potential therapeutic targets for ischemic stroke diseases.The GO enrichment analysis results showed a total of 1857 items,including BP(1668),CC(52)and MF(137).There are 148 KEGG enrichment pathways,mainly the HIF-1 signaling pathway,IL-17 signaling pathway,VEGF signaling pathway,Calcium signaling pathway,p53 signaling pathway,Toll-like signaling pathway.4.Through transcriptomics analysis,including the mRNA expression profile of the rat blank transcriptome,model transcriptome,aspirin control transcriptome,and Chuanhong Zhongfeng Capsule treatment transcriptome,the results of the study showed that the difference between the groups was significantly different.The GO enrichment analysis of the Zhongfeng Capsule treatment group showed a total of 49 items,including BP(26),CC(14),and MF(9);enrichment pathways(Homo sapiens)268;PPI protein interaction relationship showed Sichuan red There are 392 interactions in the Zhongfeng Capsule Group.Among them,the top ten are Canx(12),Cct2(21),ENSRNOG00000017595(16),ENSRNOG00000017595(13),Mrpl4(12),Nsa2(11),Rpl21(11),Rpl36(10),Rpl4(10)),Nhp2(9).The scientific research method of cross-combination of network pharmacology and transcriptome sequencing has clarified three core pathways closely related to ischemic stroke: HIF-1 signaling pathway,IL-17 signaling pathway,and p53 signaling pathway.The HIF-1 signal pathway is the signal pathway with the highest degree of enrichment among the intersection pathways.Conclusion: 1.Chuanhong Zhongfeng Capsule has a therapeutic effect on patients with ischemic stroke(blood stasis syndrome)and can improve other typical complications without adverse reactions.2.Chuanhong Zhongfeng Capsule has a significant regulatory effect on NO and SOD levels in rats with ischemic stroke,can inhibit NO levels and increase SOD levels.3.This experiment mainly adopts the research mode of combining network pharmacology and transcriptomics and clarifies that its mechanism of action may be related to the regulation of signal pathways such as HIF-1 signal pathway,IL-17 signal pathway,and p53 signal pathway.The HIF-1 signaling pathway is closely related.
