Clinical Effect Of Different Acupuncture Stimulation On Primary Dysmenorrhea Of Cold Coagulation And Blood Stasis Type

Objective:Based on the research of acupuncture and moxibustion for primary dysmenorrhea of cold blood stasis type,different acupuncture stimulation amounts were used to treat patients with primary dysmenorrhea.To observe the influence of three different acupuncture stimulation amounts(strong stimulation group,medium stimulation group and weak stimulation group)on clinical efficacy and analyze the relationship between acupuncture stimulation amount and efficacy in treating primary dysmenorrhea of cold blood stasis type.Methods:60 patients with primary dysmenorrhea of cold coagulation and blood stasis type were randomly divided into stimulation quantity a group(strong stimulation quantity),stimulation quantity B group(medium stimulation quantity)and stimulation quantity C group(weak stimulation quantity),with 20 cases in each group.1.Acupoint selection:Sanyinjiao,Diji,Guanyuan,Zhongji and Guilai(except Zhongji and Guanyuan,acupoints are selected from both sides).2.Operation method:The treatment of the three groups of patients shall be carried out independently to ensure that patients do not touch each other.After acupuncture,the patients in the three groups were treated with twisting manipulation at each point,but the amount of stimulation of twisting angle and frequency was different.① Stimulus volume group A:Use φ0.25×40mm acupuncture needle,Sanyinjiao,Diji direct needling 1 inch,Guilai,Guanyuan,Zhongji direct needling 0.5 inch.Twirling manipulation was performed at each point with a twirling angle of 180-360° and a frequency of 120-160 times/min.Each point was needled for 10s.The above twirling manipulation was performed every 10min during the needle retention period.TDP irradiates the abdomen for 30min.② Stimulus volume group B:Use φ0.25×40mm acupuncture needle,Sanyinjiao,Diji direct needling 1 inch,Guilai,Guanyuan,Zhongji direct needling 0.5 inch.Twirling manipulation was performed at each point with a twisting angle of 90-180° and a frequency of 60-120 times/min.Each point was needled for 10s.The above twirling manipulation was performed every 10min during the needle retention period.TDP irradiates the abdomen for 30min.③Stimulus volume group C:Use φ0.25×40mm acupuncture needle,Sanyinjiao and Diji acupuncture 1 inch,Guilai,Guanyuan and Zhongji acupuncture 0.5 inch.There is no manipulation after acupuncture at the above acupoints.TDP irradiates the abdomen,and the needle retention time is 30min.3.Course of treatment:Acupuncture treatment starts 5 days before menstruation,once a day,5 consecutive days as a course of treatment,1 course of treatment per menstrual cycle,3 consecutive courses of treatment,a total of 15 times.4.Outcome measures:Observe the difference of visual analogue scale(VAS)score,COX dysmenorrhea symptom scale(CMSS)score and TCM syndrome score before and after treatment.Results:53 cases were actually completed in this study,including 16 cases in stimulation quantity A group,19 cases in stimulation quantity B group and 18 cases in stimulation quantity C group.1.By comparing the VAS score,TCM symptom score and CMSS score(total frequency score and severity score of dysmenorrhea symptoms)of the two groups before and after treatment,it can be found that the VAS score,CMSS score and TCM symptom score of the three groups before and after treatment show a downward trend with time,indicating that different acupuncture stimulation doses are effective in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type,but the downward trend of the three groups is not completely consistent,Before treatment and after 3 menstrual cycles,the difference among the three groups was statistically significant(P<0.05).2.In the comparison of clinical efficacy,the total effective rates of the three groups were 93.75%in group A,73.68%in group B and 61.11%in group C.The difference was statistically significant(P<0.05).Conclusion:The same acupoints can reduce the pain index,reduce the total frequency of dysmenorrhea and improve clinical symptoms under different acupuncture stimulation.Among them,the acupuncture effect of the stimulation quantity a group(strong stimulation quantity group)is significantly better than that of the stimulation quantity B group(medium stimulation quantity group)and the stimulation quantity C group(weak stimulation quantity group),indicating that the strong stimulation quantity is closer to the optimal treatment quantity and can obtain a more satisfactory treatment effect.To sum up,the clinical therapeutic effects of different acupuncture stimulation amounts on primary dysmenorrhea of cold coagulation and blood stasis type are different.The greater the stimulation amount,the better the clinical effect.
