Objective:As one of the common and frequent gynecological diseases in clinical,the main western medicine treatment for primary dysmenorrhea is symptomatic treatment of oral drugs,such as Non-steroidal Anti-inflammatory Drugs,and there are many adverse drug reactions.Acupuncture and moxibustion has obvious efficacy in the treatment of dysmenorrhea,and can reduce the recurrence rate.Therefore,based on the theory of traditional Chinese medicine,this study discusses the therapeutic efficacy of acupuncture at Sanyinjiao,Xuehai combined with ginger separated moxibustion at Shenque,Guanyuan in the treatment of primary dysmenorrhea with cold coagulation and blood stasis type,and then evaluate the feasibility of this treatment,so as to provide a TCM external treatment scheme with more TCM characteristics and no drug side effects for this disease.Methods:64 volunteers who met the research criteria recruited by Hunan Provincial Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were randomly divided into control group and test group,with 30 cases in each group.The control group took ibuprofen tablets for treatment 1 day begin the menstrual period or after the onset of dysmenorrhea to the symptoms of dysmenorrhea disappeared.The test group received acupuncture and ginger moxibustion for treatment 5-7 days before the onset of menstruation until the onset of menstruation.Both groups of patients were treated for 3 consecutive menstrual cycles.Observe the dysmenorrhea symptom score,dysmenorrhea degree grade,TCM symptom score,VAS pain score,prostaglandins(PGF2a,PGE2)and β-Endorphin(β-EP)level to evaluate the efficacy.Results:There was no statistically significant difference between the control group and the experimental group in terms of general information and various therapeutic indicators before treatment(P>0.05);After three menstrual cycles of treatment,the dysmenorrhea symptom score,TCM symptom score,VAS pain score,and PGF2 a levels in both groups decreased,while The level of PGE2 and β-EP increased,and the change in the test group was significantly greater than the control group(P<0.05);After three treatment cycles,the test group was cured 1,significantly effective 15,effective 14,and ineffective 2,with a total effective rate of 93.0%,while the control group was cured 0,significantly effective 3,effective 25,and ineffective 4,with a total effective rate of 87.5%.The therapeutic effect of the test group was better than that of the control group(P<0.05);During a follow-up period of 3 months after discontinuation of treatment,it was found that the patient’s dysmenorrhea symptom score increased compared to that after treatment,but the test group was smaller than the control group(P<0.05).Conclusion:Acupuncture at Sanyinjiao,Xuehai combined with ginger separated moxibustion at Shenque,Guanyuan can effectively alleviate the pain symptoms and degree of pain in patients with dysmenorrhea by regulating prostaglandins(PGF2a,PGE2)and β-EP level.It has a total effective rate greater than oral western medicine.The efficacy is ideal and there are no drug side effects,which can be widely used in clinical practice.
