| Objectives:Through clinical research to observe the clinical efficacy of bingxiao powder with added flavor applied externally to prevent and treat swelling of affected limbs after total knee arthroplasty,give play to the advantages of traditional Chinese medicine,and provide basis for rapid rehabilitation of patients after total knee arthroplasty.methods:75 patients who met the criteria were randomly divided into 3 groups with 25 cases in each group.To observe the difference of clinical efficacy among the three groups,and to evaluate the clinical effect of bingxiao powder with added flavor on preventing limb swelling after total knee arthroplasty.The routine group: given the salt bag cold compress scheme,the bingnitrate powder group: 10 g borneol and 1000 g mirabilite were used for the swelling of the affected limbs.In the bingxiao powder with added flavor group,10 g borneol,1000 g mirabilite,25 g frankincense,25 g myrrh,15 g rhubarb,25 g Panax notoginseng and 10 g Phellodendron were used for swelling of affected limbs.The swelling degree of affected limbs,VAS score(in the state of rest and rehabilitation training),HSS function score of knee joint,active range of motion of knee joint and the number of patients with skin rash and pruritus were observed and recorded before operation and on the 2nd,4th,6th and 7th days after operation.The data were collected and analyzed by spss21.0statistical software.The difference was statistically significant when p <0.05.results:A total of 75 patients were included in this clinical trial and were divided into 3 groups with 25 patients in each group.There was no significant difference in age,sex,body mass index and surgical site among the three groups(P > 0.05).1.swelling degree: there was no significant difference between the three groups in the circumference 10 cm above and below the midpoint of patella before operation and the swelling value 10 cm above and below the midpoint of patella on the second day after operation(P > 0.05).On the 4th,6th and 7th day after operation,the swelling degree of affected limbs in the bingxiao powder group with added flavor was the least,and the difference was statistically significant(P < 0.05);The swelling value at 10 cm above and below the midpoint of patella in bingxiao powder group was less than that in routine group on the 7th day after operation,and the difference was statistically significant(P < 0.05).2.VAS score: there was no significant difference in VAS score among the three groups before operation,2 and 4 days after operation(P > 0.05).On the 6th day after operation,the VAS score of bingxiao powder group with added flavor was lower than that of the conventional group and the bingxiao powder group,and the difference was statistically significant(P < 0.05).Although the VAS score of the bingxiao powder group was lower than that of the conventional group at the same time point,the difference was not statistically significant(P > 0.05).At the same time point on the 7th day after operation,the VAS score of the bingxiao powder group with added flavor was lower than that of the bingxiao powder group and the conventional group(P < 0.05),and the VAS score of the bingxiao powder group was lower than that of the conventional group(P < 0.05).3.HSS score: there was no statistical significance in the comparison of HSS function scores of the three groups on the second day after operation(P >0.05).On the 4th,6th and 7th day after operation,the HSS score of bingxiao powder group with added flavor was higher than that of the conventional group and bingxiao powder group(P < 0.05);At the same time point,the HSS score of bingxiao powder group was higher than that of routine group,and the difference was statistically significant(P < 0.05).4.active range of motion of the affected knee: there was no significant difference in the range of motion of the knee in each group before operation(P > 0.05).The difference was statistically significant at 2,4,6 and 7 days after operation(P < 0.05).Comparison among groups: on the 2nd,4th,6th and7 th day after operation,the active activity of bingxiao powder group with added flavor group was the highest,the bingxiao powder group was the second,and the routine group was the lowest.The difference between groups was statistically significant(P < 0.05).5.adverse reactions: there was one case of rubella and pruritus in bingxiao powder group and bingxiao powder group with added flavor respectively.There was no statistical significance in the number of patients with rubella and pruritus among the groups(P > 0.05),and there were no other adverse reactions in each group.Conclusions:The bingxiao powder group with added flavor has a significant effect on the prevention and treatment of lower limb swelling after total knee arthroplasty.The detumescence effect of Jiawei bingnitan powder on lower limb swelling after TKA is better than that of conventional bingxiao powder.It can effectively reduce patients’ pain and has a low incidence of adverse reactions.It is worth popularizing in clinical use. |
