Clinical observation of Bingxiao Powder in the treatment of fracture and swelling of double ankle Joint Objective: to observe and analyze the clinical effect and mechanism of Bingxiao Powder on swelling and detumescence of bilateral ankle joint fracture,so as to create favorable conditions for surgical treatment of double ankle joint fracture.Methods: sixty patients with bilateral ankle joint fractures in the Department of Orthopaedics,the first affiliated Hospital of Shandong University of traditional Chinese Medicine from October 2018 to September2019 were selected and met the inclusion criteria.The rats were randomly divided into three groups: Bingxiao powder group(n = 20),ice bag group(n = 20)and control group(n = 20).After admission,all patients were given intravenous infusion of aescin sodium 10mg(Shandong Luye Pharmaceutical Co.,Ltd.,5mg: Chinese medicine H20003239)dissolved in0.9% sodium chloride injection 250 ml.The Bingxiao powder group and the ice bag group were externally applied with ice bag,while the control group was treated with sodium aescinate only by intravenous drip.Bingxiao Powder is provided by the traditional Chinese Medicine Pharmacy of the first affiliated Hospital of Shandong University of traditional Chinese Medicine.According to the proportion of 10 g borneol and 1000 g mirabilite,it is evenly mixed and packed into a cotton bag with the size of 52 cm ×42cm and five pre-packed pockets.Wrap the bag evenly around the ankle and apply it continuously for 24 hours.If the Bingxiao powder cakes or the bag hardens,replace the Bingxiao powder according to the above method,and pour out the used Bingxiao powder in a cool place to dry,recycle.The 13 cm × 16 cm rectangular ice bag made by our department was selected.The ice bag is a mixture of ice and water mixed with a lot of crushed ice,which has certain shaping ability.It is applied to the most swollen medial and lateral malleolus,30 minutes each time,three times a day,at 8 o’clock,14:00 and 20:00 every day,until the day before operation.Instruct all patients to exercise with foot and ankle pump.On the second day after admission,the patients were examined for blood routine,biochemistry,blood coagulation,blood type and infection series,and reexamined after 3 days of treatment.All patients measured the circumference of the affected limb and performed swelling score,pain score and skin wrinkles at 9: 00 a.M.every day to determine whether the conditions for surgical treatment were met.All patients underwent color Doppler ultrasound of double lower limb arteries and veins before operation to check for thrombosis.The adverse reactions of the patients were observed.The collected data were analyzed and analyzed by SPSS25.0software to evaluate the clinical efficacy of three treatment methods in eliminating swelling and pain before operation of double ankle fracture.Results: 1.Of the 60 patients selected,59 completed the clinical study.One patient in the ice bag group was removed because of thrombosis in the affected limb because of thrombosis revealed by color Doppler ultrasound examination of the arteries and veins of the lower extremities in the ice bag group.No adverse drug reactions were observed.There was no significant difference in sex,age,healthy side circumference and injured side among the three groups(P > 0.05).2.Comparison of absolute value of neutrophils: 3 days after treatment compared with before treatment: there were significant differences between Bingxiao Powder group and ice bag group and control group(P <0.05).There was no significant difference between the ice bag group and the control group(P > 0.05).Intra-group comparison: there was significant difference before and after treatment in the Bingxiao powder group(P < 0.05),but there was no significant difference between the ice bag group and the control group(P > 0.05).3.Comparison of plasma osmotic pressure: there was no significant difference in plasma osmotic pressure among the three groups(P > 0.05).4.Comparison of swelling value: there was no significant difference in the circumference of the affected side among the three groups before treatment(P > 0.05).1 comparison between groups: one day after treatment,there were significant differences between the Bingxiao powder group,the ice bag group and the control group(P < 0.05).After 2 days of treatment,compared with the ice bag group and the control group,there were significant differences between the ice bag group and the control group(P < 0.05).After 3 days of treatment,compared with the ice bag group and the control group,there were significant differences between the ice bag group and the control group(P < 0.05).Comparison between the two groups: there were significant differences among the three groups at 1 day,2 days and 3 days after treatment compared with the previous day(P < 0.05).5.Comparison of detumescence rate: there were significant differences in detumescence rate among the three groups after 3 days of treatment(P < 0.05).6.Comparison of preoperative waiting time: there was significant difference in preoperative waiting time among the three groups(P < 0.05).7.Comparison of VAS pain score: there was no significant difference in pain score among the three groups before treatment.After 1 day,2 days and 3 days of treatment,there was no significant difference between the Bingxiao powder group and the ice bag group(P > 0.05),but there was significant difference between the Bingxiao powder group and the control group(P < 0.05).8.Comparison of sleep time: there was no significant difference in sleep time among the three groups before treatment(P > 0.05).After 1day,2 days and 3 days of treatment,there was no significant difference between the Bingxiao powder group and the ice bag group(P > 0.05),but there was significant difference between the Bingxiao powder group and the control group(P < 0.05).9.Comparison of basic medical expenses: there was no significant difference in basic medical expenses among the three groups(P > 0.05).10.Bed turnover rate: assuming that all patients are externally applied ice powder,the bed turnover rate increases from 35.99% to 36.10%.10.Bed turnover rate: assuming that all patients are externally applied Bingxiao powder,the bed turnover rate increases from 35.99% to36.10%.Conclusion: Bingxiao Powder can not only significantly improve the degree of swelling of the affected limbs and increase the rate of swelling,but also has obvious advantages in the speed of swelling reduction.External application of Bingxiao Powder can significantly shorten the preoperative preparation time,reduce the hospitalization days of patients,reduce the pain and economic burden of patients,at the same time improve the soft tissue conditions of the affected limbs,and create a good foundation for follow-up surgical treatment.And save medical resources,improve bed turnover,easy to use,safe and reliable,fully reflect the characteristics of traditional Chinese medicine treatment,give full play to the advantages of traditional Chinese medicine treatment,worthy of clinical application.
