| Objective:To analyze the clinical effect of Huangqi Guizhi Wuwu Decoction on peripheral neurotoxicity induced by paclitaxel,so as to provide reliable clinical data for prevention and treatment of peripheral neurotoxicity caused by chemotherapy.Method:This is a prospective cohort study.The eligible patients were enrolled in the Oncology Department of Chengdu University of Traditional Chinese Medicine Affiliated Hospital and the Oncology Department of West China Hospital.The patients in West China Hospital were selected as control group;The patients in the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine were selected as the experimental group.The control group only received chemotherapy.Besides chemotherapy,the patients in the experimental group took Huangqi Guizhi Wuwu Decoction orally from the first day of the first chemotherapy to the end of 6cycles of chemotherapy.The degree of peripheral nerve damage in the two groups was evaluated by the patient neurotoxicity symptom score(EORTC QLQ-CIPN20)at 2,4 and 6 cycles of chemotherapy.The onset time of peripheral neurotoxic symptoms was observed and compared between the two groups.The quality of life score(EORTC QLQ-C30)and physical performance score(KPS)were used to evaluate the quality of life and physical status of the two groups of patients at 2,4and 6 cycles of chemotherapy.In addition,safety assessment is carried out by monitoring the patient’s blood routine,liver and kidney function.Results:From October 2018 to June 2020,a total of 88 patients eligible for enrollment were included,including 43 in the experimental group and 45 in the control group.(1)Symptoms of neurotoxicity(EORTC QLQ-CIPN20 score): At the end of 2,4 and 6 chemotherapy cycles,the scores of the experimental group were 21.74 ± 2.06,24.54 ± 3.49 and 27.17 ± 5.23,and the scores of the control group were 23.50 ± 2.01,26.95 ± 3.36 and 32.61 ± 5.16,with significant differences(P < 0.05),and the scores of all the experimental groups were lower than those of the control group.(2)Time of first PIPN:In both groups,the first appearance of PIPN was 59.48 ± 21.77 days in the control group and 84.15 ± 22.98 days in the test group.It indicated that PIPN occurred later in the test group than in the control group(P < 0.05).(3)Quality of life(EORTC QLQ-C30 score): The quality of life scores of the two groups of patients before chemotherapy were basically the same(P>0.05).After the end of the second cycle of chemotherapy,there was no significant difference in the scores of each dimension between the two groups(P>0.05).The physical function and overall health of the experimental group was better than the control group from the beginning of the 4th cycle to the end of the 6th cycle,and the difference was statistically significant(P<0.05).In the 6th cycle,the fatigue and pain dimensions between the two groups in the experimental group were better than those in the control group,and the difference was statistically significant(P<0.05).(4)Physical performance score(KPS score): The KPS scores between the two groups before chemotherapy and after the second cycle of chemotherapy were not statistically significant.In the 4th cycle,the KPS score of the control group was84.05 ± 8.32,and that of the experimental group was 88.38 ± 6.46(P < 0.05).The difference was statistically significant(P < 0.05).In the 6th cycle,the score of the experimental group was 82.33±8.58,and the score of the control group was76.07±9.56.Compared with the control group,the difference was statistically significant(P<0.05).Conclusion:Huangqi Guizhi Wuwu Decoction can prevent and treat PIPN and relieve the neurotoxic symptoms caused by paclitaxel chemotherapy,improve the quality of life and physical condition of patients,and relieve fatigue and pain symptoms caused by chemotherapy. |
