Background:Stroke,called stroke in traditional Chinese medicine,is the most common cerebrovascular disease in China,mostly in elderly patients.Post-stroke complications are thorny problems that endanger patients’ health and increase family and social burden.Seizures are major complications that seriously affect patients’ health after stroke,post-stroke epilepsy(Poststroke Epilepsy).PSE(abbreviated as Epilepsy for short)refers to the epileptic onset induced by stroke itself without a history of epileptic seizures before stroke or a long time after stroke,and must exclude other brain lesions or metabolic characteristics.With the aging of the population in China,there are more and more patients with stroke,and the incidence of post-stroke epilepsy is increasing year by year,which is more harmful to the health of patients and increasing the burden of families and society.therefore,people pay more and more attention to the treatment of post-stroke epilepsy.Traditional Chinese medicine has unique views and treatment methods in stroke and epilepsy,and the curative effect is remarkable,so whether post-stroke epilepsy can be treated with traditional Chinese medicine is worthy of further exploration.The pathogenesis of post-stroke epilepsy is complex and has not been fully understood,but a large number of studies have confirmed that late-onset epilepsy is closely related to poststroke inflammation and other structural changes.This study intends to use traditional antiepileptic drugs combined with traditional Chinese medicine to treat post-stroke epilepsy with wind-phlegm and blood stasis syndrome,to explore new methods for the treatment of post-stroke epilepsy,to study the efficacy of traditional Chinese medicine,and to explore the effect of traditional Chinese medicine on the inflammatory changes of post-stroke epilepsy and its possible mechanism,so as to further lay a foundation for the treatment of post-stroke epilepsy with traditional Chinese medicine.Objective: To explore the clinical efficacy of Ditan decoction and Dingxian pills combined with lamotrigine in the treatment of post-stroke epilepsy with wind-phlegm and blood stasis syndrome and its effect on inflammatory factors,and preliminarily analyze its possible mechanism.Methods: 62 patients with hemorrhage or post-infarction epilepsy who met the inclusion criteria were selected,and the differences of non-study causes were eliminated as far as possible.according to the random numerical table method,they were divided into control group(n = 31)and treatment group(n= 31).The selected oral lamotrigine was treated with routine dose,while routine stroke was given to adjust blood pressure,reduce lipid and stabilize plaque.The treatment group was treated with Ditan decoction and Dingxie pills on the basis of routine treatment in the control group,and the whole course of treatment was 3 months.The TCM clinical syndrome evaluation of epilepsy after stroke before treatment,the evaluation of National Institutes of Health Stroke scale(National Instituteof Healthstrokescale,NIHSS),inflammatory factor interleukin-6(IL-6),the number of seizures,24-hour visual screen electroencephalogram(Electroencephalogram),effective rate and various adverse reactions occurred during treatment were recorded.Results: The treatment of post-stroke epilepsy with wind-phlegm and blood stasis syndrome treated with Ditan decoction combined with lamotrigine is effective,which can improve the clinical syndrome performance of traditional Chinese medicine,reduce the clinical syndrome score and NIHSS score of traditional Chinese medicine,reduce the inflammatory response factors caused by stroke,and reduce the number of epileptic seizures and epileptic discharge.compared with lamotrigine alone,the clinical antiepileptic effect is greatly enhanced.The possible reason is that the active components of traditional Chinese medicine are involved in the mechanism of inflammatory reaction,thus reducing the level of inflammatory factors,and no clear adverse reactions and safety accidents are found in this experimental study,so it can be widely used in clinic.
