| Objective: This study intends to observe the clinical efficacy of Hutanquyu Decoction plus Reducing prescription in the treatment of patients with wind-phlegm-stasis syndrome in the acute phase of ischemic stroke and explore its potential mechanism based on network pharmacological research methods,so as to provide new ideas for the treatment of patients with acute phase of ischemic stroke with traditional Chinese medicine.Methods:(1)A total of 60 patients with wind-phlegm-stasis syndrome in the acute phase of ischemic stroke who were admitted to the Department of Encephalopathy of the First Affiliated Hospital of Hunan University of Chinese Medicine from April 2018 to October2019 and who met the inclusion criteria were randomly divided into the control group and the treatment group according to their medical treatment order.The control group received basic western medicine treatment,and the treatment group was in the control group on the basis of the basic treatment added to take the decoction of Eliminating phlegm and removing stasis.Data were collected before and after treatment.The total curative effect,clinical neurological function defect score,fasting blood glucose,blood lipid and TCM syndrome score of apoplexy in the two groups were statistically processed,and the results of the two groups were compared to evaluate the efficacy and safety of the treatment.(2)Using TCMSP relevant literature retrieval and phlegm and remove stasis soup and related targets,the main active compounds through the standard name Uniprot database acquisition targets,through Gene Cards database access to the related targets of the disease through the STRING database and Cytoscape 3.6.1 software builds relevant core target protein interactions with the disease of PPI network.Core targets were enriched by GO function analysis and KEGG pathway enrichment analysis using R language software.Results:(1)Clinical Observation and Research Center: Clinical neurological deficit score: the difference between before and after treatment was statistically significant(P<0.05),and the score of the treatment group was lower than that of the control group.After 2weeks of treatment,there were differences in fasting blood glucose and TC between the two groups(P<0.05),and the treatment group was lower than the control group.TG,ldl-c and hdl-c showed no difference(p>0.05).Comparison of fasting blood glucose and blood lipid between the two groups before and after treatment: fasting blood glucose,TC,ldl-c and hdl-c in the treatment group before and after treatment had statistical significance(P<0.05),but TG had no statistical significance(P >0.05).TCM syndrome score: There was a statistical difference in TCM syndrome score between the two groups before and after treatment(P<0.01);The decrease was more obvious in the treatment group than in the control group(P<0.01).Total curative effect: compared with the treatment group,the curative effect of the two groups is more significant.(2)207 effective compounds and 176 drug targets were obtained according to the screening conditions,and 48 core targets were selected according to the average degree value of 7.43.GO functional enrichment analysis resulted in 89 items(P<0.05),and KEGG pathway enrichment analysis resulted in 158 pathways related to the disease(P<0.05).Conclusion:(1)Phlegm and dispelling stasis soup can obviously change of neural function in patients with acute ischemic stroke,TCM syndrome integral,and reduce the fasting blood glucose,total cholesterol levels,and had no adverse reaction occurred during treatment,can significantly improve patients with nerve function and living ability,has certain fall blood sugar blood fat,safe and reliable treatment,side effects,suitable for clinical further promotion and application,and provides new thinking for clinical treatment of acute ischemic stroke.(2)Active compounds in the phlegm and dispelling stasis soup may by hepatitis b signaling pathway and MAPK signal pathway,AGE-RAGE signal pathway,TNF,IL-17 signaling pathways,signal channel Th17 cell differentiation signaling pathways,c-type lectin receptors signaling pathways,endocrine resistance signaling pathways,cell apoptosis signaling pathways,the way such as toll-like receptors signaling pathways in JUN,MAPK3,MAPK1,TNF,IL6 and AKT1,ESR1 core targets on the health effects,It suggests that Huphlegm-Quyu Decoction has a potential correlation and therapeutic effect with the inflammatory reaction,atherosclerosis,nerve cell apoptosis,immune response and other biological processes in the pathogenesis of ischemic stroke,which provides a certain theoretical basis for the clinical application of this drug and subsequent mechanism research. |
