Objective: Chronic obstructive pulmonary disease(COPD)is a common preventable and treatable respiratory disease characterized by persistent airflow limitation that is progressive and associated with enhanced chronic inflammatory responses of the airways and lungs to noxious particulate matter or gases.At present,the pathogenesis of COPD has not been determined,mainly related to inflammatory response,oxidative stress,protease/antiprotease imbalance;modern medicine mainly aerobic therapy,control of infection,bronchiectasis,anti-inflammatory spasmolysis and other treatment methods.Traditional Chinese medicine attributes COPD to "lung distension",and phlegm and blood stasis are its main pathogenic factors and pathological products.Traditional Chinese medicine(TCM)can relieve the clinical symptoms of patients and improve activity endurance and quality of life through simple and easy external treatment such as oral administration of traditional Chinese medicine or acupuncture,application,and atomization of traditional Chinese medicine.The aim of this study was to investigate the effect of Huatan Huoxue Jiangqi Decoction on clinical efficacy and inflammation and coagulation in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients with phlegm-dampness obstructing lung syndrome.Methods: From October 2020 to October 2021,80 patients with acute exacerbation of chronic obstructive pulmonary disease of phlegm-dampness obstructing lung syndrome were randomly divided into control group(n=40)and treatment group(n=40).The control group was treated with standardized western medicine.The treatment group was treated with Huatan Huoxue Jiangqi Decoction on the basis of the control group.Finally,a total of 39 cases in the control group and 38 cases in the treatment group.The changes of clinical symptom scores,assessment test(CAT),modified British medical research council(m MRC),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10),white blood cell(WBC),NEUTtrophil(NEUT),C-reactive protein(CRP),amyloid protein SAA(SAA),fibrinogen(FIB)and D-dimer(D-D)levels were compared between the two groups at admission and before discharge.Results:1.Both the control group and the treatment group could reduce the clinical symptom score,CAT score and m MRC grade(P<0.05);after treatment,the clinical symptom score and CAT score in the treatment group decreased more significantly than those in the control group(P<0.05).2.Both the control group and the treatment group could significantly reduce the serum levels of cytokines IL-6,IL-8,IL-1 β and TNF-α(P<0.05),and the levels of these four cytokines in the treatment group decreased more significantly than those in the control group after treatment(P<0.05);both the control group and the treatment group could significantly increase the serum IL-10 level(P<0.05),and the serum IL-10 level in the treatment group increased more significantly than that in the control group after treatment(P<0.05).3.Both the control group and the treatment group could significantly reduce the serum WBC,NEUT,CRP and SAA levels(P<0.05).After treatment,the serum WBC,NEUT,CRP and SAA levels in the treatment group decreased more significantly than those in the control group(P<0.05).4.Both the control group and the treatment group could significantly reduce the serum FIB and D-D levels(P<0.05);after treatment,the serum FIB and D-D levels in the treatment group decreased more significantly than those in the control group(P<0.05).Conclusion: Conventional western medicine treatment based on Huatan Huoxue Jiangqi Decoction can significantly reduce the airway inflammation in AECOPD patients with phlegm-dampness obstructing lung syndrome,improve cough,expectoration,wheezing and other respiratory symptoms,improve the quality of life of patients;more significantly improve the blood hypercoagulability of patients and prevent thrombosis.
