| Objective To observe the clinical efficacy of acupuncture combined with Wenyang Yishen moxibustion in the treatment of chronic non-specific low back pain of cold-dampness type,to explore its mechanism and clinical application advantages, and to provide reference for acupuncture combined with moxibustion to treat chronic non-specific low back pain of cold-damp type.Method A total of 66 patients with chronic non-specific low back pain of cold-damp type who visited the acupuncture and moxibustion department of Hubei Provincial Hospital of Traditional Chinese Medicine from October 2020 to October 2021 were collected. There were 33 cases in each group, the acupuncture combined with Wenyang Yishen moxibustion group(Observation group)and the conventional acupuncture group(control group). The acupuncture points of the two groups were Shenshu(double),Dachangshu(double),Yinlingquan(double),Yaojiaji(double),Weizhong(double),Kunlun(double),Yaoyangguan,Ashi points. They were treated once every other day, three times a week, six times a course of treatment, two consecutive courses of treatment a total of 12 times. The acupuncture and moxibustion group received Wenyang Yishen moxibustion after the first acupuncture every week, namely, the first, fourth, seventh, and tenth acupuncture treatments, respectively, for a total of 4 times.During the research process, the gender, age, course of disease, pain visual analog scale score(Visual Analogous Scale,VAS),TCM syndrome score and dysfunction index(Oswestry Disability Index,ODI)before the first treatment and after the last treatment and adverse events 、ase dropout were recorded accurately and in detail. After all the data were collected, statistical methods were used to analyze the data between and within the group,to compare the efficacy of acupuncture combined with Wenyang Yishen moxibustion and conventional acupuncture in the treatment of chronic non-specific low back pain of cold-dampness type.Result1.A total of 66 patients were included in this study, with 33 cases in the acupuncture and moxibustion group and 33 cases in the acupuncture group.During the whole research process,1 case dropped out in each of the two groups, and a total of 64 cases were finally included in the data analysis, including 32 cases in the acupuncture and moxibustion group and 32 cases in the acupuncture group.2.Baseline data comparison: the gender, age, course of disease,VAS score,ODI index, and TCM syndrome score before the first treatment in the acupuncture and moxibustion group and the acupuncture group were all P>0.05,indicating that the differences between the two groups were all equal. Not statistically significant and comparable.3.Comparison of VAS scores: Comparison within groups: The VAS scores of the two groups of patients after treatment were significantly lower than those before treatment, and P<0.01,indicating that both treatments can significantly reduce the symptoms of low back pain.Comparison between groups: The VAS scores of the two groups of patients after treatment and the difference of VAS scores before and after treatment were compared with each other, and it was found that both P>0.05,indicating that there was no significant difference in the efficacy of the two groups.4.Comparison of ODI index: Comparison within group: Paired sample t test was used to compare the ODI index of the two groups of patients before and after treatment, all P<0.01.Comparison between groups: After all treatments, the independent samples t test was used to compare the ODI index after treatment and the difference of ODI index before and after treatment between the two groups, all P<0.01.5.Comparison of TCM syndrome scores: Comparison within the group: After two courses of treatment, the symptoms of cold and dampness in the two groups were significantly improved(P<0.05).Comparison between groups: The score of cold-damp syndrome in the acupuncture and moxibustion group decreased more significantly than that in the acupuncture group(P<0.01).6.Comparison of total effective rate: After 2 courses of treatment, the effective rate of the acupuncture and moxibustion group was 96.875%,of which 8 cases were cured,15 cases were markedly effective,7 cases improved, and 1 case was invalid; The total effective rate of the acupuncture group was 87.5%,of which3 cases were cured,12 cases were markedly effective,13 cases were improved and 4 cases were ineffective. The efficacy of the two groups by the rank sum test,P<0.05,the difference was statistically significant. Conclusion Acupuncture combined with Wenyang Yishen moxibustion and acupuncture alone have curative effects on chronic non-specific low back pain of cold-dampness type, but the therapeutic effect of acupuncture combined with Wenyang Yishen moxibustion is better.Although there is no significant difference between the two in reducing pain in patients with chronic non-specific low back pain of cold-dampness type, acupuncture combined with Wenyang Yishen moxibustion is significantly better in improving lumbar spine function,cold-dampness type TCM syndrome, and improving the quality of life of patients. In addition,as a new type of moxibustion technology,Wenyang Yishen moxibustion have wide area,Enough quantity,Long interval, can warm the yang and invigorate the kidney, warm the meridians and clear the collaterals, dispel cold and remove dampness. It has the advantages of being green,safe,non-invasive and painless, and easy for patients to accept. It is worthy of further research and promotion. |
