The Correlation Of AHSG/APN Ration In Typer2 Diabetes Mellitus With Peripheral Vascualar Disease

Objective: Merge of carotid atherosclerosis in patients with type 2diabetes mellitus risk factors,analysis of AHSG/APN ratio of the value of diagnosis of type 2 diabetes mellitus and carotid plaques.Methods: Collect the first hospital affiliated to hebei institute of north in March 2020-December 2020 109 cases of type 2 diabetes patients in hospital,and after a rigorous screening the number of eligible for study at the same period 100 cases and 30 cases of healthy check-up.Patients were divided into three groups according to IMT: type 2 diabetes mellitus(T2DM-A group),type 2 diabetes mellitus with carotid intima thickening group(T2DM-B group),type 2 diabetes mellitus with carotid plaque formation group(T2DM-C group)and normal control group(NC).Ask four groups of the general conditions,the object of study while detecting fasting plasma glucose(FPG),glycosylated hemoglobin(Hb Alc),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting insulin(FINS),fetuin A(AHSG)and level of adiponectin(APN),calculate the indexes of insulin resistance(HOMA IR),body mass index(BMI)and fetuin/adiponectin ratio(AHSG/APN).The correlation between the above indexes and AHSG,APN and AHSG/APN was analyzed by Pearson correlation analysis.Risk factors for T2 DM with peripheral vascular disease were evaluated using Logistic regression analysis.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of AHSG,APN and AHSG/APN in the assisted diagnosis of T2 DM with peripheral vascular disease.Results:(1)Four groups of serum AHSG,APN,AHSG/APN leve l comparison: compared to a control group,diabetes group AHSG an d AHSG/APN ratio are increased,and the levels of serum APN is re duced,statistically difference(P<0.05);Between the four groups of A HSG two compare to each other,the differences were statistically sig nificant(P<0.05);At the same time,with the increase of IMT value,the level of APN decreased gradually.Compared with the NC group,the changes of APN level in T2DM-A group >T2DM-B group >T2DM-C group were different(P<0.05).Compared with T2DM-A groupand T2DM-B group,the level of serum APN in T2DM-A group wa s the lowest,and the differences were statistically significant.Althoug h the level of serum APN in T2DM-A group > T2DM-B group,ther e was no statistical significance between the two groups(P>0.05).Th e serum AHSG/APN ratio was mainly as follows: NC group There was no significant difference between T2DM-B group and T2DM-A group(P>0.05),but there was statistical difference between T2DM-Bgroup and NC group(P<0.05).There was no significant difference between T2DM-A group and NC group(P>0.05).(2)The results of rson correlation analysis showed that AHSG wa s positively correlated with disease course,BMI,SBP,DBP,FPG,H b A1 c,TG,LDL-C,FINS,HOMA-IR,IMT and AHSG/APN ratio,andnegatively correlated with hdl-c and APN.APN was negatively corr elated with disease duration,BMI,SBP,DBP,FPG,Hb A1 c,TG,LD L-C,FINS,HOMA-IR,IMT,AHSG,AHSG/APN ratio,and positivel y correlated with HDL-C.AHSG/APN ratio was not correlated with age(P>0.05),but was correlated with other indexes(P<0.05).(3)Logist regression results suggested that the risk factors of type 2diabetes mellitus with peripheral vascular disease might be disease course,SBP,AHSG/APN,and the increase of DBP in a certain range was a protective factor.(4)ROC curve results showed that AHSG/APN ratio auxiliary diagnosis of type 2 diabetes mellitus and peripheral vascular lesions area under curve(AUC)is greater than the AHSG and APN,the AUC value is0.828,0.850,0.878,respectively,the sensitivity is respectively 94%,72%,88%,specificity of 56.2%,88.7% and 73.8% respectively,to comprehensively consider the diagnostic performance of AHSG/APN ratio is better than that of AHSG and APN.Conclusion:(1)The serum fetoglobin A level of type 2 diabetic patients in this study population was significantly higher than that of normal people,and the serum adiponectin level was significantly lower than that of normal people.(2)In this study population,the occurrence of type 2 diabetes carotid plaques and the longer the course of type 2 diabetes mellitus,systolic blood pressure,the greater the ratio is higher and AHSG/APN.(3)This study suggests the AHSG AHSG/APN ratio and APN ind ex can better reaction type 2 diabetes mellitus and carotid plaques.
