| Objective:Through clinical controlled trials,to observe the therapeutic effect of contralateral needling combined with resistance acupuncture in the treatment of post-stroke shoulder hand syndrome through clinical controlled trials. So as to seek a more safe and effective treatment, and provide a basis for clinical application of this method in the treatment of shoulder hand syndrome after stroke. Methods:Selecting the patients hospitalized in the six wards of acupuncture and moxibustion in the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from May2020 to February 2021.72 patients with stage I and II shoulder hand syndrome after stroke were randomly divided into observation group and control group with 36 cases in each group. Both groups of patients were treated with routine medical treatment and “xingnao kaiqiao acupuncture by traditional Chinese medicine master Shi Xue-min.On this basis, the control group was treated with ordinary acupuncture. The observation group was treated with contralateral needling combined with resistance acupuncture. The two groups were treated once a day,6 times a week for 2 weeks. The scores of shoulder hand syndrome assessment scale(SHSS),simplified Fugl-Meyer motor function scale(FMA),visual analogue scale(VAS)and swelling degree were recorded before and after treatment.According to the improvement of pain, edema and upper limb motor function, the curative effect of the two groups were analyzed.Results:1.There was no significant difference in general information(gender, age, stroke type)between the two groups(P > 0.05);Before treatment, there was no significant difference in SHSS score,FMA score,VAS score and swelling degree score between the two groups(P>0.05),and they were comparable.2.After 2 weeks of treatment, the SHSS score,VAS score and swelling degree score of the two groups were decreased compared with before treatment(P < 0.05).It shows that the two groups of therapy can improve the sensation, movement and autonomic nerve of patients with shoulder hand syndrome after stroke. The FMA score of the two groups was higher than before treatment(P < 0.05).It means the two groups of therapy are helpful to the recovery of upper limb motor function.3.There were significant differences in SHSS score,FMA score,VAS score and swelling degree score between the two groups after treatment(P < 0.05).Conclusion:1.Acupuncture from contralateral tiaokou to chengshan combined with resistance acupuncture and ordinary acupuncture both can improve the symptoms of sensory, motor and autonomic nerve in patients with shoulder hand syndrome after stroke, and improve the motor function of upper limbs.2.Compared with the ordinary acupuncture, acupuncture from contralateral tiaokou to chengshan combined with resistance acupuncture has more advantages in relieving shoulder pain, enhancing motor function of affected limb and improving edema.3.Contralateral needling combined with resistance acupuncture is safe and effective. And it is worthy of clinical promotion and further research. |
