| Objective:Hand eczema(HE)is a common inflammatory disease in dermatology.It is refractory and complex,which seriously affects the quality of life of patients and causes huge economic burden.In order to identify the triggers and reduce the recurrence rate of hand eczema,we recruit 97 patients with hand eczema in Suining for allergens by patch test and serum specific IgE,and analyse the correlation of the pathogeny,allergens,gender,age,occupation,severity,and other factors.Methods:Druing the period from October 2019 to January 2021,97 patients with hand eczema from the Department of Dermatology of Suining Central Hospital were screened according to inclusion and exclusion criteria.And their general conditions were recorded,including age,gender,duration,occupation,time of wearing rubber gloves,frequency of hand washing,et al.And finally,candidates were examined with patch test and serum specific IgE.Results:Among the 97 patients with hand eczema,the ratio of male and female was1:1.62,the average age was 35.78±13.54 years,and the median duration was 2(1～5)years.36.08%of all patients had atopic history,and there were no significant difference in atopic history between different gender.The main types of occupation were cosmetologists,housewives,office clerks,students,and cement workers.The time of wearing gloves,access to water and the frequency of hand washing of women were significantly more than men(P<0.05).Among 97 patients with hand eczema tested with patch test,52(53.61%)patients showed at least one positive reaction.The most common allergens were nickel sulfate,potassium dichromate,thiomersal,aromatic mixture and cobalt chloride.And allergic to nickel sulfate mainly were cosmetologists;allergic to potassium dichromate mainly were cement workers;allergic to thiomersal were housewives,medical personnel,cement workers.Besides,the male with positive of potassium dichromate were significantly higher than female(P<0.05).Age and atopic history didn’t affect the positive rate of patch test in patients with hand eczema(P>0.05).Among 97 patients with hand eczema tested with serum specific IgE,47(48.45%)patients showed at least one positive reaction.The most common inhalation allergens were dermatophagoides farinae,dermatophagoides pteronyssinus and house dust.The most common edible allergens were eggs,milks and wheat flours.And allergic to dermatophagoides farinae and dermatophagoides pteronyssinus mainly were office clerks;allergic to eggs mainly were children.There was significant difference in the positive rate of serum specific IgE between different age(P<0.05),and positive rates of house dust,eggs and milk in children were significantly higher than those in young and old.Gender and atopic history didn’t affect the positive rate of serum specific IgE in patients with hand eczema(P>0.05).The average of HECSI was 16.14±7.68.The median of DLQI was 8(5～10).The score of HECSI and DLQI were positive correlation with age,duration,the time of access to water(r_s>0,P<0.05).And a statistically significantly positive correlation between DLQI and HECSI was found.Conclusion:The major patients with hand eczema in Suining are female,with an average age of 35.78 years old.And the most common professions among them are cosmetologists,housewives,office clerks,students,cement workers and construction workers.The most common contact allergens are nickel sulfate,potassium dichromate,thiomersal,aromatic mixture and cobalt chloride.The most common inhalation allergens are dermatophagoides farinae,dermatophagoides pteronyssinus and house dust.The most common edible allergens are eggs,milks and wheat flours.Hypersensitivity to potassium dichromate is more common in men,and allergies to dermatophagoides pteronyssinus,milk,eggs are more common in children.The HECSI score and DLQI score are affected by the age,duration and the time of access to water,what’s more,the higher severity index of skin lesions patients get,the lower quality of life they are. |
