Background:Cardiovascular and cerebrovascular diseases are accompanied by many symptoms,complicated etiology,low immunity and tolerance and easy to relapse.By2018,China’s cardiovascular disease patients reached 290 million,15 million people die of cardiovascular disease every year,suffering from cerebrovascular disease reached 180 million,every year about 3 million people die of cerebrovascular disease.In order to reduce the morbidity and mortality of cardiovascular and cerebrovascular diseases,active symptomatic treatment is needed,and more importantly,the factors affecting cardiovascular diseases are systematically evaluated.Traditional risks(such as obesity,hypertension,hyperlipidemia,hyperglycemia,smoking,insufficient physical activity,unreasonable diet,and atmospheric pollution)will increase the risk of cardiovascular and cerebrovascular diseases.Non-traditional risk factors(marriage)are also the high risk factors for cardiovascular and cerebrovascular diseases.However,the relationship between marital status and cardiovascular and cerebrovascular diseases is not clear at present,and the research on the correlation between the specific status of marital status(single,divorced,and widowed)and cardiovascular and cerebrovascular diseases is rare.Therefore,through meta-analysis,we intended to evaluate all the published related articles to clarify the correlation between marital status and the occurrence and prognosis of cardiovascular and cerebrovascular diseases,so as to provide a theoretical basis for the clinical prevention and treatment of patients with cardiovascular and cerebrovascular diseases.Methods:In order to obtain systematic and comprehensive literature on the relationship between marital status and cardiovascular and cerebrovascular diseases,we conducted a comprehensive search on Cochrane Liabrary,Pubmed,Web of science,Embase,CNKI Data Master Database,Wanfang Database Knowledge Service Platform,etc.The search time was from the establishment of the database to December 2020.The search strategy used subject words plus free words,and the English search words were marital status,married,unmarried,widowed,divorced,seperate,single,motability,coronary heart disease,heart death,cardiovascular disease,stroke,heart attack,prospective cohort studies;Chinese key words are:marital status,marriage,unmarried,single,divorce,widowhood,coronary heart disease,stroke and cardiovascular disease.Outcome indicators:coronary heart disease death,stroke death,all-cause death,coronary heart disease,stroke and cardiovascular disease.The relative risk(RR)and 95%confidence interval(95%CI)were calculated for statistical analysis.Results:Through systematic search,and according to the inclusion and exclusion criteria,39 articles were finally included,and a total of 2244739 patients were included.Meta-analysis indicated that,compared to married patients,unmarried patients had statistically significant outcomes as follows:death from coronary heart disease(RR1.32,95%CI 1.24–1.40,P<0.00001,I~2=0%),death from stroke(RR 1.21,95%CI1.10–1.32,P<0.0001,I~2=0%),all-cause death(RR 1.23,95%CI 1.11-1.36,P<0.0001,I2=62%),coronary heart disease(RR 1.31,95%CI 1.20-1.42,P<0.00001,I~2=25%),and stroke(RR 1.57,95%CI 1.47-1.66,P<0.00001,I~2=0%).Singleness and the incidence of coronary heart disease(RR 1.20,95%CI 1.05-1.37,p=0.008,I~2=39%)were statistically significant for a given marital status.The incidence of divorce and death from coronary heart disease(RR 1.13,95%CI 1.02-1.25,p=0.02,I~2=0%)was statistically significant.The incidence of widowhood and stroke(RR2.18,95%CI 1.98–2.41,P<0.00001,I~2=0%)were statistically significant.Conclusions:Marital status affects the occurrence and prognosis of cardiovascular disease,and should be considered in the risk assessment for cardiovascular disease.In the future,more large prospective cohort studies are needed to further explore the relationship between marital status and cardiovascular and cerebrovascular disease. |
